研究目的：

此实验目的是探索腰椎融合术后居家康复的优化方式，帮助基层缓解康复资源不足的问题，解决四级网络康复中居家康复的实施困难问题。节约腰椎融合术康复成本，提高患者康复治疗积极性，提高腰椎融合术患者康复疗效，增加腰椎融合术社区康复的资源，帮助解决康复供需矛盾，最终使康复工作真正步入社区走进家庭。  

Objectives of Study：

The purpose of this experiment is to explore the optimal way of home rehabilitation after lumbar fusion, help the grassroots to alleviate the problem of inadequate rehabilitation resources, and solve the difficulty of home rehabilitation implementation in the four-level network rehabilitation. It can save the rehabilitation cost of lumbar fusion, improve the enthusiasm of patients for rehabilitation treatment, improve the rehabilitation effect of patients with lumbar fusion, increase the resources of community rehabilitation of lumbar fusion, help solve the contradiction between supply and demand of rehabilitation, and finally make the rehabilitation work truly enter the community into the family.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

( 1 ) 年龄＞18 岁；( 2 )性别不限；( 3 )术后14天内即转入我院的康复患者 ；( 4 )具有自主行为能力；( 5 ) 知情同意；( 6 )可定期接受康复指导、康复治疗及随访者；( 7)患者内固定稳定良好。

Inclusion criteria

(1) Age > 18 years old; (2) There is no limitation on gender; (3) Convalescent patients transferred to our hospital within 14 days after surgery; (4) It has the capacity of independent behavior; (5) Informed consent; (6) Can receive rehabilitation guidance, rehabilitation treatment and visitors regularly; (7) The patient's internal fixation was stable.

排除标准：

( 1 ) 多发性骨折；( 2 ) 合并感染者；( 3 ) 合并骨质疏松；( 4 ) 拒绝加入本研究；( 5 ) 合并严重血栓；( 6 ) 合并严重的心脑血管并发症；

Exclusion criteria：

(1) Multiple fractures; (2) Co-infected persons; (3) Combined with osteoporosis; (4) Refuse to join the study; (5) Severe thrombosis; (6) Complicated with serious cardiovascular and cerebrovascular complications.
