研究目的：

本研究旨在构建基于可穿戴设备并以PPM模型为框架的2型糖尿病医院-家庭过渡期管理方案。探讨该方案对2型糖尿病患者血糖指标（糖尿病血红蛋白水平、目标范围内时间（TIR））、自我管理行为、用药依从性的干预效果。  

Objectives of Study：

The purpose of this study is to construct a hospital-family transition management plan for type 2 diabetes based on wearable devices and within the framework of PPM model. To explore the intervention effect of this scheme on blood glucose indexes (diabetic hemoglobin level, target time (TIR)), self-management behavior and medication compliance in patients with type 2 diabetes mellitus.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①符合《中国 T2DM 防治指南（2020年版）》诊断标准，经临床医师诊断为2型糖尿病且明确诊断时间≥6 个月；②年龄≥18岁；③意识清醒且无认知功能障碍，能够配合完成研究；④能够使用智能手机；⑤自愿参与本研究。

Inclusion criteria

①In accordance with the diagnostic criteria of Chinese guidelines for prevention and treatment of T2DM (2020 edition), type 2 diabetes was diagnosed by clinicians and the diagnosis time was more than 6 months;②age ≥ 18 years old; ③ conscious and without cognitive impairment, able to cooperate with the study; ④ able to use smart phones;?⑤volunteer to participate in this study.

排除标准：

①合并重度水肿、感染、末梢循环障碍的患者；②合并严重心血管疾病、肾脏疾病及恶性肿瘤患者；③孕妇及哺乳期妇女；④患有听力或视力障碍患者。

Exclusion criteria：

①patients with severe edema, infection and peripheral circulation disturbance;② patients with severe cardiovascular disease, kidney disease and malignant tumor;③ pregnant and lactating women, and ④hearing or vision impairment.
