研究目的：

探讨在俯卧位腰椎患者手术中，潮气量负荷试验对每搏量变异度预测患者容量反应性的影响。  

Objectives of Study：

To investigate the effect of tidal volume challenge on the prediction of volume responsiveness of patients with stroke volume variability undergoing prone lumbar surgery

药物成份或治疗方案详述：

体位转为俯卧位后，监测 6ml/kg潮气量下的每搏出量变异率（SVV 6 ）。然后进行潮气量负荷试验，即将潮气量（V T ）由 6 ml/kg 增加至 8 ml/kg，维持 1min 后监测 8ml/kg潮气量下的每搏出量变异率（SVV 8 ）。同时计算潮气量负荷试验前后每搏出量变异率变化值即ΔSVV（ΔSVV = SVV 8 – SVV 6 ）。完成潮气量负荷试验后将潮气量调回 6ml/kg 进行机械通气，一分钟后对患者进行补液试验（以 6ml/kg 的胶体输注 20 分钟），观察补液试验 5 分钟后心输出指数（cardiac index，CI）的变化，若补液试验后心输出指数增加幅度≥15%，则患者为补液试验有反应者，反之为补液试验无反应者，以此为标准将患者分为容量反应有反应组和容量反应无反应组。 

Description for medicine or protocol of treatment in detail：

After the prone position, the stroke volume variability (SVV 6 ) were recorded, then TVC was performed, which increased V T from 6 ml/kg to 8 ml/kg predicted body weight for 1 minute, and the SVV 8 were recorded. The variation of SVV (ΔSVV) were calculated after TVC (ΔPPV =ΔSVV = SVV 8 – SVV 6 ). When TVC was completed, the V T was reduced back to 6ml / kg predicted body weight for fluid challenge. Increase in cardiac index after fluid challenge ≥15% was considered to be a positive response. Patients were divided into groups of responders or non-responders.  

纳入标准：

择期行腰椎后路内固定手术患者 (1)年龄18-75岁；(2)美国麻醉医师协会（ASA ）分级Ⅰ～Ⅲ级；(3)体重指数（BMI）18-30 kg/m2；（4）术前无肺气肿、肺大疱等严重肺部疾病。

Inclusion criteria

Patients were selected for elective spinal surgery in prone position. (1)aged 18-75 years (2)American Society of Anesthesiologists physical status Ⅰ to Ⅲ,(3)with body mass index of 18-30 kg/m 2 ;(4)without severe lung diseases such as emphysema or bullae before surgery.

排除标准：

(1)合并重要器官功能障碍者；(2)右心功能不全、心律失常、心脏瓣膜病、心脏内的分流；(3)存在严重呼吸系统疾病，如慢性阻塞性肺疾病、胸腔积液、哮喘、气胸等；（4)正在参与其他临床试验和拒绝参与试验者。

Exclusion criteria：

(1)With organ dysfunction;(2)having right ventricular dysfunction, cardiac arrhythmias, valvular heart disease, intracardiac shunt, (3)having severe respiratory disease,chronic obstructive pulmonary disease(COPD),pleural effusion,asthma,pneumothorax;(4)Participants in other clinical trials and those who refuse to participate in the trial.
