研究目的：

通过临床试验验证无创通气模式治疗下的氧反馈调节功能（IOC）在临床使用中的安全性、有效性。评价两种模式在初始呼吸支持中减少医护人员操作及降低患儿使用呼吸机时间、减少并发症的疗效差异。  

Objectives of Study：

To verify the safety and efficacy of oxygen feedback regulation (IOC) under non-invasive ventilation mode in clinical use. To evaluate the difference in the efficacy of the two modes in reducing staff operation and ventilator time and complications in initial respiratory support.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①胎龄30周～34周，体重小于1800g
②符合《实用新生儿学》 ( 第 5版) NRDS 诊断 标准，临床拟诊断为NRDS（结合临床症状以及胸部X线表现）
③出生时直接予无创呼吸机辅助通气，或已经行气管插管或因紧急复苏行短暂气管插管者，有创呼吸机撤离后予无创呼吸机辅助呼吸者均可入组
④父母签署知情同意书
⑤性别不限

Inclusion criteria

①Gestational age 30 to 34 weeks, weight less than 1800g
② Meet the N RDS diagnostic criteria of Practical Neonatology (5th edition), and the clinical diagnosis is N RDS (combined with clinical symptoms and chest X-ray findings)
③ Those who are directly given non-invasive ventilator assisted ventilation at birth, or who have undergone endotracheal intubation or have undergone short endotracheal intubation for emergency resuscitation, and who are given non-invasive ventilator assisted breathing after evacuation of invasive ventilator can be enrolled
④Parents signed the informed consent form
⑤Sex is not limited

排除标准：

①呼吸系统发育异常，如鼻部发育畸形、腭裂、小颌畸形综合征、后鼻孔闭锁、食管-气管漏、先天性膈疝等；进行性呼吸衰竭不能维持氧合、严重的BPD的患儿。
②心血管系统不稳定，如肺动脉高压、低血压和心功能不全；
③呼吸驱动不稳定，如中枢性呼吸暂停或无自主呼吸；
④肺气肿、气胸、消化道出血、严重腹胀、NEC，局部损伤（鼻黏膜、口腔、面部）；
⑤病情加重需要插管机械通气维持的患儿；
⑥放弃治疗，自动出院和死亡的患儿
⑦父母拒绝签署治疗同意书
⑧正在参加其他临床试验的患儿

Exclusion criteria：

① Abnormal development of the respiratory system, such as nasal development malformation, cleft palate, minor jaw malformation syndrome, posterior nostrise atresia, esophageal-tracheal leakage, congenital diaphragmatic hernia, etc.; progressive respiratory failure can not maintain oxygenation, severe BPD children.
②Unstable cardiovascular system, such as pulmonary arterial hypertension, hypotension, and cardiac insufficiency;
③Respiratory drive instability, such as central apnea or no spontaneous breathing;
④Emphysema, pneumothorax, gastrointestinal bleeding, severe abdominal distension, NEC, local damage (nasal mucosa, mouth, and face);
⑤Children with disease aggravation requiring mechanical ventilation and maintenance for intubation;
⑥Abandof treatment, automatic discharge and death of children
⑦Parents refused to sign the treatment consent form
⑧Children are participating in other clinical trials

设定一个固定的FiO2 ，当患儿的SpO2在90%-94%以上或以下，血氧报警，则医护人员将手动区调节氧浓度，以维持SpO2在 90%-94%之间。

干预措施：

开启呼吸机自带的智能氧控功能， FiO2 自 动调节初始区间为0. 21 ～ 0. 40，后续根据实际情况再做调整， SpO2 控制区间为 90% － 94%之间，加用呼吸机自带的监护仪探头绑在患儿四肢处，当患儿经皮血氧饱和度下降至90%以下或上升至94%以上时， FiO2将自动调节数值，以维持血氧在90%-94%之间 。若FiO2已经上升至最大值仍不能维持设定的氧和，则可以调整FiO2 的自动调节区。

Intervention：

Turn on the intelligent oxygen control function of the ventilator, FIO2 automatic adjustment of the initial interval of 0.25% . Twenty-one-zero. 40. The control range of SpO2 is between 90% and 94% . The monitor probe of the ventilator is attached to the limbs of the child, fIO2 is automatically adjusted to maintain oxygen levels between 90% and 94% when the Oxygenation level drops below 90% or rises above 94% . If FiO2 has risen to the maximum and can not maintain the set oxygen sum, the FIO2 autoregulation region can be adjusted.
