研究目的: |
本研究拟利用人工智能和多模态MRI建立前列腺癌根治性大分割放疗疗效的预测模型。该模型一方面可以筛选出放疗疗效好的患者,给予根治性放疗,联合内分泌治疗时间可在指南范围内适当缩短,避免过度治疗造成对生活质量的影响及额外的经济负担;另一方面可以筛选出放疗效果不佳的患者,这部分患者可考虑调整放疗方案或合并相对长程内分泌治疗或手术,从而为不同类型的局限期前列腺癌患者提供个体化的最佳治疗选择。同时构建前列腺及危及器官外轮廓的自动精准靶区勾画模型,提高医生工作效率,进一步推动前列腺癌放疗靶区的标准化。 我们将人工智能技术应用到局限期前列腺患者的多模态MRI数据,研发出预测根治性放疗疗效的智能决策系统,不断进行验证和优化,推进普惠医疗体制的建立,从而使我国有限医疗资源的应用更加有效和合理。 |

Objectives of Study: |
This study intends to use artificial intelligence and multimodal MRI to establish a prediction model for the curative effect of radical large segment radiotherapy for prostate cancer. On the one hand, the model can screen out patients with good radiotherapy effect and give them radical radiotherapy. The time of combined endocrine therapy can be appropriately shortened within the scope of the guidelines to avoid the impact of over-treatment on the quality of life and additional economic burden; On the other hand, we can screen out patients with poor radiotherapy effect. For these patients, we can consider adjusting the radiotherapy plan or combining with relatively long-term endocrine therapy or surgery, so as to provide individualized and optimal treatment options for different types of patients with limited stage prostate cancer. At the same time, an automatic and accurate target area delineation model of prostate and organs endangering the external contour is constructed to improve the work efficiency of doctors and further promote the standardization of prostate cancer radiotherapy target area. We apply AI technology to multimodal MRI data of prostate patients in limited period, develop an intelligent decision-making system to predict the curative effect of radical radiotherapy, continuously verify and optimize, and promote the establishment of inclusive medical system, so as to make the application of limited medical resources in China more effective and reasonable. |

1: T1-4N0M0 prostate cancer patients 2: Biopsy pathological diagnosis of prostate adenocarcinoma, Gleason score 6-10 3: Received radical hypofractionated image-guided IMRT, Dt60Gy/20f or Dt70Gy/28f, etc. 4: Neoadjuvant endocrine therapy 1-3 months before radiotherapy 5: Pelvic MRI including T1, T2, DWI, CE sequence in our hospital before treatment |
