研究目的：

运用干预模式“VIP for future care”结合决策树-马尔科夫模型进行预立医疗照护计划的卫生经济学评价。  

Objectives of Study：

The intervention model "VIP for Future Care" combined with decision tree-Markov model was used to evaluate the health economics of advance care planning.

药物成份或治疗方案详述：

拟将入住医院肿瘤科的200位患者随机分为试验组和对照组。对照组给予常规病情沟通。试验组除按对照组干预外，运用“VIP for future care”模式干预。比较2组患者在干预前、干预后及随访的医疗费用、临终治疗意愿、生存质量等。  

Description for medicine or protocol of treatment in detail：

A total of 100 patients who were admitted to the oncology department of the hospital were randomly divided into experimental group and control group. The control group was given routine disease communication. In addition to the control group, the experimental group used the "VIP for future Care" mode of intervention. The medical expenses, end-of-life treatment intention and quality of life of patients in 2 groups were compared before intervention, after intervention and during follow-up.  

纳入标准：

①年龄为 60 岁及以上；②晚期肿瘤患者(癌症 诊断为Ⅲ、Ⅳ期)；③蒙特利尔认知评估量表(MoCA)评分≥26 分；⑤知晓病情。

Inclusion criteria

① The age is 60 and above; ② Patients with advanced tumors (diagnosed as stage ⅲ and ⅳ); ③ Montreal Cognitive Assessment Scale (MoCA) score ≥26; Know the illness.

排除标准：

①有精神障碍或疾病者； ②不能耐受长时间交流者； ③患者或家属任何一方拒绝参加者。

Exclusion criteria：

① People with mental disorders or diseases; ② Can't tolerate long time communication; ③ Either the patient or the family rejects the participant.
