研究目的：

通过将细针穿刺芬太尼-布比卡因等比重腰麻应用于脊柱结构异常患者下肢骨科手术中，观察患者是否会出现椎管内麻醉相关的并发症尤其是神经系统并发症，从而证明细针穿刺芬太尼-布比卡因等比重腰麻在脊柱结构异常患者下肢骨科手术中应用的安全性和有效性，在未来对于全身状况较差并且合并有心肺系统疾病的脊柱结构异常的患者能够应用此技术，从而减少患者的围术期的并发症和住院费用。  

Objectives of Study：

Fine needle puncture fentanyl-bupivacaine isobaric spinal anesthesia was applied to lower limb orthopedic surgery in patients with spinal structural abnormalities to observe whether there would be complications related to intraspinal anesthesia, especially nervous system complications. it is proved that fine needle puncture fentanyl-bupivacaine isobaric spinal anesthesia is safe and effective in lower limb orthopedic surgery in patients with spinal structural abnormalities. In the future, this technique can be applied to patients with poor general condition and spinal structural abnormalities complicated with cardiopulmonary diseases, thus reducing perioperative complications and hospitalization costs.

纳入标准：

宁夏回族自治区人民医院2023年02月-2023年09月需在腰麻下行下肢骨科手术的患者，年龄18-100岁，ASA：I-III级，对本研究内容及意义知情，并签署知情同意书

Inclusion criteria

Patients in the People's Hospital of Ningxia Hui Autonomous Region who need lower extremity orthopedic surgery under lumbar anesthesia from February 2023 to September 2023, aged 18-100 years old, ASA: grade I-III, who are aware of the content and significance of this study, and sign the informed consent

排除标准：

既往有精神疾病、中枢神经系统疾病、重症肌无力、自身免疫性疾病；有凝血功能障碍、穿刺部位感染或全身感染、存在脊柱术后或脊柱结构异常未行矫正术引起的神经系统并发症；术前因昏迷、严重痴呆、语言障碍、终末期疾病等无法交流、脊柱受伤72小时以内者；病情危重（术前ASA分级≥IV级者）；严重肾功能损害（需行肾脏替代治疗者）；严重肝功能损害（Child-Pugh分级C级者）；严重心功能不全（NYHA≥Ⅲ级者）；主治医生或研究者认为存在其他不宜参加本研究的情况；拒绝参加本研究者以及患者家属中途退出本次试验者。

Exclusion criteria：

Previous mental disorders, central nervous system diseases, myasthenia gravis, autoimmune diseases; blood coagulation disorders, puncture site infection or systemic infection, nervous system complications caused by spinal surgery or spinal structural abnormalities; those who were unable to communicate before operation due to coma, severe dementia, language disorder, end-stage diseases, and spinal injury within 72 hours. Critical condition (preoperative ASA grade ≥ IV); severe renal function damage (need renal replacement therapy); severe liver function damage (Child-Pugh grade C); severe cardiac insufficiency (NYHA ≥ grade Ⅲ); other conditions that the attending doctor or researcher considered unsuitable to participate in this study; those who refused to participate in this study and their families.
