研究目的：

（1）构建以5E康复模式为框架的老年脑梗死患者早期康复护理方案并评价对患者的身心影响；（2）为临床护理人员提供了一个有效的选择，作为传统护理方案的补充，以更好地满足这一人群的需求。  

Objectives of Study：

1.To construct an early rehabilitation nursing plan for older patients with cerebral infarction based on the 5E rehabilitation model and evaluate the physical and mental impact on patients;2.It provides an effective choice for clinical nursing staff, as a supplement to the traditional nursing scheme, to better meet the needs of this group of people.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）符合《中国急性缺血性脑卒中诊治指南2014》的诊断标准；（2）年龄≥60岁者；（3）首次发病；（4）发病后出现偏瘫症状；（5）意识清楚；（6）知情同意并签署知情同意书。

Inclusion criteria

1.meeting the diagnostic criteria of "China Guidelines for the Diagnosis and Treatment of Acute Ischemic Stroke 2014"; 2. age ≥ 60 years; 3. first onset; 4. hemiplegia symptoms after onset; 5. clear consciousness; 6. informed consent and signed informed consent.

排除标准：

（1）病情危重；（2）其他原因造成肢体功能障碍；（3）伴有重大疾病（心、肝、肾等脏器疾病）；（4）正在参加其他临床试验；（5）不同意出院随访者。

Exclusion criteria：

1.critical illness; 2. limb dysfunction caused by other reasons; 3. accompanied by major diseases (heart, liver, kidney, and other organ diseases); 4. participating in other clinical trials; 5. disagreeing to discharge follower.
