研究目的：

试图证实慢性颞叶癫痫并发抑郁的患者外周血中IDO激活及IDO激活后外周血和尿液中色氨酸代谢产物3-HAA、3-HK浓度增加和KYN/TRY、QUIN/KYNA比值升高，粪便中肠道菌群改变，并且这些生化标记与抑郁评分相关。从临床患者身上证实IDO激活后色氨酸代谢产物3-HAA、3-HK浓度增加和KYN/TRY、QUIN/KYNA比值升高，肠道菌群改变是癫痫相关的抑郁的原因和生化标记。通过证实此通路，了解颞叶癫痫相关性抑郁的机制，并为癫痫相关性抑郁的诊断和防治提供理论依据。  

Objectives of Study：

The purpose of this study was to confirm that the concentrations of tryptophan metabolites 3-HAA and 3-HK in peripheral blood and urine and the ratio of KYN/TRY to QUIN/KYNA in peripheral blood and urine increased after IDO activation and IDO activation in patients with chronic temporal lobe epilepsy complicated with depression, and these biochemical markers were related to depression score. It was confirmed from clinical patients that the concentration of tryptophan metabolites 3-HAA, 3-HK and the ratio of KYN/TRY to QUIN/KYNA increased after IDO activation, and the change of intestinal flora was the cause and biochemical marker of epilepsy-related depression. By confirming this pathway, we can understand the mechanism of temporal lobe epilepsy-related depression and provide the theoretical basis for the diagnosis, prevention, and treatment of epilepsy-related depression.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄18-65岁，无性别限制；
2. 符合经典的 1989 年国际抗癫痫联盟关于癫痫和癫痫综合征的分类标准：
（1）临床发作症状提示致痫病灶位于颞叶；
（2）发作期或发作间期脑电图（electroencephalograph ，EEG）提示痫性病灶位于颞叶或起源于颞叶；
（3）未接受过抗抑郁药物；
3. 抑郁发作诊断标准符合精神疾病诊断与统计手册第4版（The Diagnostic and Statistical Manual of Mental Disorders IV，DSM-IV）中分类与相关诊断标准；
4. 汉密尔顿抑郁量表（HAMD）17 项评分≥17分且<24分；
5. 所有受试者或其监护人知情同意，愿意参与并签署知情同意书。

单一的颞叶癫痫患者目前诊断当前应符合1、2条件。
颞叶癫痫伴发抑郁的患者目前诊断应符合1、2、3、4条件；且诊断为伴有抑郁发作（包括初次发作和复发）而未服用药物；癫痫诊断早于初次抑郁发作半年以上。
正常对照组：正常的健康志愿者。半年内无痫性发作和抑郁发作出现。

Inclusion criteria

1. Age 18-65 years, no gender restriction;
2. Meet the classic 1989 International League Against Epilepsy classification criteria for epilepsy and epileptic syndromes:
(1) Clinical seizure symptoms suggesting that the epileptogenic lesion is located in the temporal lobe;
(2) Electroencephalograph (EEG) during the seizure or interictal period suggests that the epileptic lesion is located in or originates from the temporal lobe;
(3) No antidepressant medication was received;
3. The diagnostic criteria for depressive episodes met the classification and related diagnostic criteria in the Diagnostic and Statistical Manual of Mental Disorders IV (DSM-IV);
4. Hamilton Depression Scale (HAMD) 17-item score >=17 points and <24 points;
5. All subjects or their guardians gave informed consent and were willing to participate and sign an informed consent form.

Single patients with temporal lobe epilepsy with a current diagnosis should meet conditions 1 and 2.
Patients with temporal lobe epilepsy with depression should have a current diagnosis that meets conditions 1, 2, 3, and 4; a diagnosis of concomitant depressive seizures (both initial and recurrent) without medication; and a diagnosis of epilepsy that predates the initial depressive seizure by more than six months.
Normal control group: normal healthy volunteers. No epileptic seizures and depressive episodes occurred within six months.

排除标准：

1. 有严重内科疾病患者（包括心血管、消化系统、泌尿系统、免疫系统、呼吸系统等）；
2. 有炎症性疾病和免疫系统疾病患者；
3. 有药物滥用病史；
4. 有其他精神疾病，例如精神分裂症、双相障碍、饮食失调症等；
5. 已使用细胞因子类药物或影响细胞因子之类的药物及其他干扰色氨酸代谢通路的药物。

Exclusion criteria：

1. Patients with serious medical diseases (including cardiovascular, digestive, urological, immune, respiratory, etc.);
2. Patients with inflammatory and immune system diseases;
3. Patients with a history of substance abuse;
4. Patients with other psychiatric diseases, such as schizophrenia, bipolar disorder, eating disorders, etc.;
5. Patients who have used cytokine-like drugs or drugs that affect cytokines and such and other drugs that interfere with Tryptophan metabolic pathway drugs.
