研究目的:

在IVF-ET(in vitro fertilization and embryo transfer)/ICSI-ET(Intracytoplasmic sperm injection and embryo transfer)治疗周期中,利用多模态人类胚胎整倍体性AI(Artificial Intelligence)预测模型对高龄不孕患者所获得的囊胚进行非侵入性AI实时筛选与移植,探讨该AI模型是否能够提高高龄不孕女性取卵后首次囊胚移植的持续妊娠率、活产率以及降低流产率。  

Objectives of Study:

Using a multi-modal AI model for predicting human embryo ploidy, blastocysts obtained from infertile aged patients during IVF treatment are non-invasively and real-time evaluated. The study aims to explore whether this AI model can improve the ongoing pregnancy rate and live birth rate, and reduce the miscarriage rate after the first blastocyst transplantation in the study population.

药物成份或治疗方案详述:

评估利用多模态人类胚胎整倍体性AI预测模型对高龄不孕女性进行IVF-ET/ICSI治疗所获得的囊胚进行实时筛选与移植与常规人工筛选胚胎行移植两组患者移植后临床妊娠率、活产率和流产率。 

Description for medicine or protocol of treatment in detail:

To evaluate the clinical pregnancy rate, live birth rate and miscarriage rate after blstocyst transfer based on the AI assisted embryo selection model and conventional embryo grading in IVF/ICSI cycles among infertile women of advanced age. 

纳入标准:

1、具有IVF或ICSI指征的夫妇,女性年龄在35-42岁之间,BMI在18-30kg/m2 范围内;
2、行第一或第二周期IVF-ET/ICSI-ET治疗的不孕女性;
3、采用常规控制性卵巢刺激方案;如拮抗剂方案、激动剂方案或黄体期促排卵方案;
4、获卵数≥6枚;
5、正常受精胚胎持续在time-lapse中培养;
6、同意D3全部养囊,且D5/6/7形成囊胚≥2个,且形态学评级为4BC/4CB以上 ;
7、对本研究充分知情理解,并签署知情同意书。

Inclusion criteria

1.Couples with IVF / ICSI indications ,female aged between 35-42 years old and BMI in the range of 18-30kg/m2;
2.Infertile female who underwent the first or second cycle of IVF-ET/ ICSI-ET treatment;
3.Routine controlled ovarian stimulation with long GnRH agonistGnRH antagonist protocol or luteal phase ovarian stimulation;
4.The number of oocytes obtained ≥ 6;
5.Normal fertilized embryos are continuously cultured in time-lapse;
6.All D3 cultured into blastocysts, and D5 / 6 / 7 blastocysts ≥ 2, and the morphological rating was above 4BC / 4CB;
7.Fully informed understanding of this study and signed informed consent.

排除标准:

1、行PGT治疗的不孕患者;
2、子宫纵隔、单角子宫、双子宫等生殖道畸形者;
3、宫腔粘连、宫腔积液、多发性子宫内膜息肉、粘膜下子宫肌瘤或薄型子宫内膜(胚胎移植日内膜厚度<7mm者);
4、未进行输卵管结扎或堵塞治疗的输卵管积水患者;
5、自身免疫或内分泌疾病病史患者:系统性红斑狼疮(SLE)、未分化结缔组织病(抗ANA阳性)、甲亢、甲减等;
6、反复种植失败(移植3次及以上高质量胚胎未孕)或不明原因早期妊娠丢失≥2次患者;
7、夫妻双方至少有一方存在IVF或ICSI禁忌证,如控制不良的I型或II型糖尿病、未明确病因的肝脏疾病或肝功能异常、肾脏疾病或肾功能异常、严重贫血、深静脉血栓病史、肺栓塞病史、脑血管意外病史、控制不良的高血压或诊断明确的心脏病、宫颈癌、内膜癌或乳腺癌病史、不明原因的阴道出血、严重的精神与神经性疾病等。

Exclusion criteria:

1.Infertility patients treated with PGT;
2.Uterine septum, unicornuate uterus, double uterus and other reproductive tract malformations;
3.Intrauterine adhesions, uterine effusion, multiple endometrial polyps, submucous uterine fibroids or thin endometrium ( embryo transfer day endometrial thickness < 7mm );
4.Patients with hydrosalpinx who were not treated with tubal ligation or blockage;
5.Patients with a history of autoimmune or endocrine diseases : systemic lupus erythematosus ( SLE ), undiferentiated connective tissue disease( anti-ANA positive ), hyperthyroidism, hypothyroidism, etc.
6.Recurrent implantation failure(no pregnancy after 3 or more high quality embryo transfer)or unexplained early pregnancy loss at least twice;
7.At least one of the couples has IVF or ICSI contraindications ( such as poorly controlled type I or type II diabetes unexplained liver disease or liver dysfunctionrenal disease or renal dysfunctionsevere anemiahistory of deep vein thrombosis, pulmonary embolism, cerebrovascular accidentpoorly controlled hypertension or diagnosed heart diseasehistory of cervical cancer, endometrial cancer or breast cancerunexplained vaginal bleeding, etc ).

盲法:

本试验拟采用单盲的试验方法进行,即对受试者设盲,直至出现主要终点事件或试验结束后再对受试者揭盲。不论研究组还是对照组,均进行单囊胚培养和冻存,均进行AI辅助整倍体囊胚筛查与AI优先移植排序。得到AI辅助筛选胚胎评级结果后,研究组患者依据“AI辅助筛选胚胎标准” 确定首次移植胚胎,对照组患者依据胚胎形态确定首次移植胚胎。

Blinding:

Blinding is only set up for the subjects in this trial. The blinding will be revealed to the subjects after the occurrence of a major endpoint event or at the end of the trial. Both the study group and the control group will undergo single blastocyst culture and freezing, and the embryos will be evaluated for ploidy using the AI model. After obtaining the embryo grading results, the first embryo transplantation will be based on the results from AI-assisted ploidy evaluation in the study group or based on conventional Gardner criteria in the control group.
