研究目的：

本研究探讨密闭式氦氧通气前后老年高血压机械通气患者损伤肺组织炎症反应、膈肌功能的变化，并进一步评估氦氧通气在较高呼气末正压（PEEP）水平情况下随通气时间延长对老年高血压患者血流动力学、肺血管活性因子、脑氧饱和度（rSO2）、脑血流及神经营养因子的影响，验证氦氧通气能改善老年高血压机械通气患者的心肺脑功能，以期将成为重症心肺脑康复有效的重要治疗手段。  

Objectives of Study：

This study was designed to investigate the changes of inflammatory reaction of injured lung tissue and diaphragm function in elderly hypertensive patients with mechanical ventilation before and after closed helium oxygen ventilation, and further evaluate the effects of helium oxygen ventilation on hemodynamics, pulmonary vasoactive factors, cerebral oxygen saturation (rSO2), cerebral blood flow and neurotrophic factors in elderly hypertensive patients with higher positive end expiratory pressure (PEEP) as ventilation time prolonged.It is verified that helium oxygen ventilation can improve the cardiopulmonary brain function of elderly hypertensive patients with mechanical ventilation, so as to become an effective treatment method for severe cardiopulmonary brain rehabilitation.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①高血压病符合《中国高血压防治指南2020》诊断标准；
②年龄≥65岁；
③因肺炎需有创机械通气治疗，呼吸机参数吸入氧浓度控制在40％左右脉搏血氧饱和度始终维持95%以上；
④进行血流动力学监测。

Inclusion criteria

①Hypertension was diagnosed according to the Chinese guidelines for the prevention and treatment of hypertension 2020. ②The age is ≥ 65 years old.
③Invasive mechanical ventilation is required due to pneumonia. The ventilator parameter inhalation oxygen concentration is controlled at about 40% and pulse oxygen saturation is maintained at more than 95% all the time.
④Hemodynamic monitoring is carried out.

排除标准：

继发性高血压，心律失常，严重心力衰竭（NYHA心功能分级III-IV级），休克，肺部恶性肿瘤，操作未得到患者及家属同意者。

Exclusion criteria：

Secondary hypertension, arrhythmia, severe heart failure (NYHA cardiac function grade III-IV), shock, pulmonary malignant tumor.The operation is not approved by patients and their families.
