研究目的：

本研究旨在比较以奥马环素和替加环素为基础的抗感染治疗方案对HAP/VAP患者的临床疗效是否有差异，拓展奥马环素的院内感染适应症。  

Objectives of Study：

The aim of this study was to compare whether there is a difference in the clinical efficacy of anti-infective regimens based on omadacycline and tigecycline in patients with HAP/VAP and to expand the nosocomial infection indications for omadacycline.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

符合以上《中国成人医院获得性肺炎与呼吸机相关性肺炎诊断和治疗指南（2018年版）》中HAP/VAP诊断标准；微生物培养结果提示XDR耐药菌。

Inclusion criteria

Meet the diagnostic criteria for HAP/VAP in the above Guidelines for the diagnosis and treatment of hospital-acquired pneumonia and ventilator-associated pneumonia in adults in China (2018 edition); microbiological culture results suggest XDR-resistant bacteria.

排除标准：

（1）血培养阳性者；（2）颅脑疾病累及呼吸中枢，不能维持自主呼吸节律者；（3）患有神经肌肉疾病导致呼吸肌无力者；（4）细菌培养结果为铜绿假单胞菌、变形杆菌属和普罗维登斯菌属等；（5）入组前使用过奥马环素和/或替加环素抗感染方案；（6）妊娠期及哺乳期女性；（7）年龄＜18岁。

Exclusion criteria：

(1) those with positive blood cultures; (2) those with cranio-cerebral disease involving the respiratory center and unable to maintain voluntary respiratory rhythm; (3) those with neuromuscular disease causing respiratory muscle weakness; (4) those with bacterial culture results of Pseudomonas aeruginosa, Aspergillus spp. and Providence spp. etc.; (5) those who had used omadacycline and/or tigecycline anti-infection regimens before enrollment; (6) women during pregnancy and lactation; (7) those aged <18 years.

随机方法（请说明由何人用什么方法产生随机序列）：

随机数字表法，由总负责人马莉，按照试验要求进行随机分配。参照统计学教科书上的随机数字表，可根据试验的的组数来决定分配方案，约60例患者，按照纳入前后顺序，进行从1-60的编号，组数为2，查阅随机表，以第二行第五列为起始点，以此向第五列往下，收集连续的60个随机数字，对应纳入患者的编号，随机数为奇数的为以奥马环素为基础的抗感染治疗组，偶数的为以替加环素为基础的抗感染治疗方案组。
