研究目的：

探讨不同方式的人工辅助咳嗽刺激技术对脑干卒中患者咳嗽功能障碍的临床疗效。观察不同的人工手法辅助咳嗽刺激技术的技术特点，以及该技术对于脑干卒中患者治疗的安全性及临床可行性，并将实验结果进行临床应用推广。  

Objectives of Study：

Explore the clinical efficacy of different artificial assisted cough stimulation techniques on cough dysfunction in brainstem stroke patients. Observe the technical characteristics of different manual assisted cough stimulation techniques, as well as the safety and clinical feasibility of this technique for the treatment of brainstem stroke patients, and promote the clinical application of the experimental results.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合中国脑血管疾病分类2015版关于脑卒中损伤的诊断和分类标准，经头颅CT或MRI检查确诊为脑干损伤，首次发病，病程≤3个月； 2.患者能理解和遵循简单的口头指令（MMSE评分:中学以上>24分、小学>20分、文盲>17分）； 3.年龄40岁~75岁之间； 4.首次发病且在3个月之内； 5.咳嗽峰值流速＜270L/min； 6.患者及家属愿意接受床旁人工辅助咳嗽训练，并签署知情同意书。

Inclusion criteria

①1.Conforming to the diagnostic and classification criteria for stroke injury in the 2015 edition of the Chinese Classification of Cerebrovascular Diseases, confirmed as brainstem injury through head CT or MRI examination, with the first onset and a course of ≤ 3 months; 2. Patients are able to understand and follow simple oral instructions (MMSE score: above middle school>24 points, primary school>20 points, illiteracy>17 points); 3.Between the ages of 40 and 75; 4. First onset within 3 months; 5. Cough peak flow rate<270L/min; 6.The patient and their family members are willing to receive bedside assisted cough training and sign an informed consent form.

排除标准：

1.既往有癫痫史，有严重的心、肺、肝、肾等重要脏器功能衰竭者； 2.支气管胸膜瘘、气胸、肺出血及咯血者； 3.患者及其家属不接受治疗； 4.存在影响咳嗽功能的其他神经、呼吸、骨科疾病，如膈神经损伤、哮喘、肋骨骨折、严重骨质疏松。

Exclusion criteria：

1. Individuals with a history of epilepsy and severe failure of important organs such as the heart, lungs, liver, and kidneys; 2. Patients with bronchopleural fistula, pneumothorax, pulmonary hemorrhage, and hemoptysis; 3. The patient and their family members do not receive treatment; 4. There are other neurological, respiratory, and orthopedic diseases that affect cough function, such as phrenic nerve injury, asthma, rib fractures and Severe osteoporosis.

由研究者事先拟定33个研究对象序号，后按照计算机随机数字表的方法将入选受试者按随机数由大到小的方式随机分为三组（排名前11位为舌后刺激组、中间11位为气管刺激组、后11位为膈肌刺激组）。

The researchers prepared 33 study object numbers in advance, and then randomly divided the selected subjects into three groups according to the method of computer random number table (the top 11 were the retrolingual stimulation group, the middle 11 were the tracheal stimulation group, and the last 11 were the diaphragm stimulation group).
