研究目的：

1. 探索慈心与正念观呼吸团体家庭训练对青少年非自杀性自伤行为的疗效。 2. 探索慈心与正念观呼吸团体家庭训练对具有非自杀性自伤行为脑功能，神经内分泌功能，基因甲基化水平的影响。  

Objectives of Study：

1.Exploring the effectiveness of the group family training in loving-kindness meditation and breathe-focused meditation on nonsuicide self-injury in adolescents. 2.Exploring the effects of the group family training in loving-kindness meditation and breathe-focused meditation in brain function, neuroendocrine function, and gene methylation levels.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.过去1年以内至少存在5次以上自伤行为的青少年，且评估前一个月内至少才能在1次自伤行为； 2.年龄在12-18岁，性别不限； 3.使用KSAD-PL诊断伴或不伴抑郁障碍 ； 4.首发未服药，且愿意未来三个月内暂时不进行任何药物治疗；或已经使用药物治疗（限于SSRI类抗抑郁药物，及第二代抗精神病药），但连续4周内无药物剂量及种类的调整，且未来三个月内暂时无药物治疗调整计划； 5.右利手； 6.无幽闭恐惧症，身体内无金属植入物

Inclusion criteria

1. Adolescents who have engaged in at least 5 acts of self-harm within the past year, and have engaged in self-harm at least once within the month prior to assessment; 2. Aged between 12 and 18, regardless of gender; 3. Diagnosed with or without a comorbid depressive disorder using the KSAD-PL; 4. Not currently taking medication and willing to refrain from any medication treatment for the next three months, or currently taking medication (limited to SSRIs and second-generation antipsychotics) without any changes in dosage or type for four consecutive weeks and with no planned medication adjustments for the next three months; 5. Right-handed; 6. No fear of confinement and no metal implants in the body.

排除标准：

1.器质性精神障碍，双相情感障碍，精神活性物质所致的精神障碍，精神分裂症及其谱系障碍，躯体疾病所致精神障碍； 2.当前存在严重的自杀意念及自杀行为，（使用自杀想法和意念评估量表评估，以下问题其中之一得分3分或以上（满分4分，得分越高程度越重） “您觉得未来产生自杀想法的可能性有多大”，或者“您觉得未来会制定自杀计划的可能性有多大”，“您觉得未来会尝试自杀的可能性有多大”；）需要住院治疗，或家属认为当前非自杀性自伤行为不能接受，要求住院治疗需要住院治疗； 3.存在听力障碍或言语理解困难；当前正在接受任何形式的心理治疗（个体心理治疗或团体心理治疗）； 4.当前正在接受物理治疗（如经颅磁刺激）；

Exclusion criteria：

1. Organic mental disorders, bipolar disorder, mental disorders caused by psychoactive substances, schizophrenia and its spectrum disorders, and mental disorders caused by physical diseases; 2. There are currently serious suicidal ideation and behaviors, (using the Suicide ideation and ideation assessment scale, one of the following questions is scored 3 points or more (out of 4 points, the higher the score, the heavier the degree) "How likely do you think there is to be suicidal ideation in the future", "How likely do you think there is to be a suicide plan in the future", "How likely do you think there is to be a suicide attempt in the future";) Requiring hospitalization for treatment, or family members who believe that current non suicidal self injury behavior is unacceptable and require hospitalization for treatment; 3. Having hearing impairment or difficulty understanding language; Currently receiving any form of psychotherapy (individual psychotherapy or group psychotherapy); 4. Currently undergoing physical therapy (such as transcranial magnetic stimulation);

将使用Matlab进行区块随机化，生成计算机生成的随机代码，随机化将由独立的统计工程师进行，独立统计师不会进入入组评估，提供治疗或者参与后测评估，因此将独立于评估和治疗人员。

Matlab will be used for block randomization to generate computer generated random code. Randomization will be carried out by independent statistical engineers. 盲法：

Blinding：

There is a single line of communication between the coordination liaison and the doctors responsible for enrollment and follow-up evaluation, as well as the psychotherapists and patients involved in intervention. The doctors responsible for enrollment and evaluation do not participate in treatment and are blinded in grouping patients; And require patients not to disclose their treatment group information when receiving evaluations. The psychotherapist in charge of treatment does not participate in the evaluation and requires patients not to disclose their scores on various scales when receiving treatment. The final report of the study will be based on the results of blind evaluation. At the same time, doctors who maintain a single blind evaluation will fill in a blind fidelity questionnaire to monitor whether the blind method is fully implemented. Due to the need for patients to participate in clinical trials of psychotherapy, blind methods cannot be maintained for patients.
