探索引起青年人萎缩性胃炎的高危因素，针对青年人提出治疗及预防胃癌癌前病变的方案。  

Objectives of Study：

To explore the high-risk factors causing atrophic gastritis in young people, and propose treatment and prevention programs for precancerous lesions of gastric cancer in young people.

药物成份或治疗方案详述：

胃癌的发生是一个慢性的、多因素参与的过程，一般认为胃癌前的级联发展进程为“慢性萎缩性胃炎-肠上皮化生-低级别瘤变/低级别异型增生-高级别瘤变/高级别异型增生”。萎缩性胃炎及肠化生作为胃癌前病变及胃癌前状态的主要表现，是胃癌进展通路上的重要组成部分。胃癌前病变常发生于45岁以上的中老年人，我国临床指南推荐常规胃镜检查年龄为40岁，青年胃癌前病变则不多见。然而，近年来青年中有治疗必要癌前病变的发病率正逐渐升高。目前亚洲各国结合地区上消化道疾病发病率，专家共识及临床指南建议胃镜初始筛查年龄为40-50岁。对于小于40岁的青年人群，现有的研究及专家共识暂时无相关的标准诊疗诊治方案。相比中老年人中胃癌病变的表现，青年人胃镜下病变部位以远端胃居多，多见于胃角与胃窦，贲门与胃底部发生率低，与老年人胃癌前病变正好相反，这种病变是否与胃窦来源的慢性萎缩性胃炎相关，目前仍未有明确的临床及检测结果支撑。本研究旨在探索青年人中胃癌前病变的高危因素，以对青年人群提出避免胃癌前病变进展的建议。 

Description for medicine or protocol of treatment in detail：

The occurrence of gastric cancer is a chronic and multifactorial process, and it is generally believed that the progression of the pre gastric cancer cascade is "chronic atrophic gastritis, intestinal metaplasia, low grade neoplasia or low grade dysplasia, high grade neoplasia or high grade dysplasia". Atrophic gastritis and intestinal metaplasia, as the main manifestations of precancerous lesions and precancerous status, are important components of the progression pathway of gastric cancer. Gastric precancerous lesions often occur in middle-aged and elderly people over the age of 45. Our clinical guidelines recommend that the age of conventional gastroscopy be 40 years old, while gastric precancerous lesions are rare in young people. However, recently, the incidence rate of precancerous lesions that are necessary for treatment among young people is gradually increasing. At present, according to the incidence rate of upper gastrointestinal diseases in Asia, expert consensus and clinical guidelines suggest that the initial screening age of gastroscopy should be 40-50 years old. For young people under the age of 45, there is currently no relevant standard diagnosis and treatment plan based on existing research and expert consensus. Compared to the manifestations of gastric cancer in middle-aged and elderly people, the majority of lesions under gastroscopy in young people are located in the distal stomach, mostly in the gastric horn and antrum, and the incidence of cardia and fundus is low, which is the opposite of precancerous lesions in the elderly. Whether this lesion is related to chronic atrophic gastritis originating from the antrum is still not supported by clear clinical and testing results. The purpose of this study is to explore the high risk factors for precancerous lesions in young people, and to provide recommendations for young people to avoid the progression of precancerous lesions. 

纳入标准：

1: 病例组：就诊于指定医院胃镜中心，胃镜结果提示萎缩性胃炎，肠化生的人群 2: 病例组：年龄18-40岁（包括18岁及40岁） 3: 对照组：按照1:2年龄、性别匹配，就诊于指定医院胃镜中心，胃镜结果未提示萎缩性胃炎，肠化生的人群

Inclusion criteria

1: Case group: people who visited the gastroscopy center of the designated hospital and the gastroscopy results showed atrophic gastritis and intestinal metaplasia 2: Case group: aged 18-40years (including 18 and 40 years old) 3: Control group: according to 1:2 age and sex matching, people who visited the gastroscopy center of the designated hospital, and the gastroscopy results did not indicate atrophic gastritis and intestinal metaplasia

排除标准：

1: 严重的肝、肾和心肺功能障碍

2: 使用非甾体抗炎药（NSAIDs）治疗

3: 研究开始前4周内使用了抗血小板与抗凝类、类固醇、抗生素类药物；

4: 研究开始前已诊断为癌症、胃切除术

5: 既往患胃或十二指肠溃疡

6: 患有血液疾病，包括贫血和其他疾病导致的出血倾向

Exclusion criteria：

1: Severe liver, kidney, and cardiopulmonary dysfunction

2: Use of non-steroidal anti-inflammatory drugs (NSAIDs)

3: Use of antiplatelet and anticoagulant, steroid, and antibiotic drugs within 4 weeks before the start of the study

4: Before the start of the study Diagnosed with cancer, gastrectomy

5: suffered from gastric or duodenal ulcers in the past

6: suffered from blood diseases, including anemia and bleeding tendency caused by other diseases
