研究目的：

比较急性消化性溃疡出血高危患者急诊内镜组织胶水注射治疗与标准内镜治疗两种治疗方法的安全性与有效性。  

Objectives of Study：

To compare the safety and efficacy of emergency Medical Glue Embolization type injection and standard endoscopic therapy in patients with high risk of acute peptic ulcer bleeding

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）年龄为18-75周岁，男女不限；具有呕血和/或黑便，具有失血的表现如头昏、出汗、心慌，休克等； 2）各种病因所致消化性溃疡并急性出血，内镜下见喷射状出血（Forrest Ia）、活动性渗血（Forrest I b）、见裸露血管直径≥2mm (Forrest IIa级), 只要满足其中一条即可； 3）再出血风险高（定义为Blatchford评分≥6分）； 4）签署知情同意书。

Inclusion criteria

1)Men and women aged 18-75 are not limited; Hematemesis and/or black stool, with blood loss, such as dizziness, sweating, palpitation and shock; 2)Peptic ulcer complicated with acute hemorrhage caused by various causes can be seen as jet hemorrhage (Forrest Ia) and active bleeding (Forrest Ib) under endoscope, and the diameter of exposed blood vessels is ≥ 2 mm (Forrest IIa grade), as long as one of them is met; 3)High risk of Recurrent Bleeding (defined as Blatchford score ≥ 6; 4)Sign the informed consent form

排除标准：

1）高度怀疑溃疡恶变的患者； 2） 合并其他肿瘤患者； 3）门脉高压静脉曲张出血的患者； 4）严重的心肺及脑血管疾病，包括急性心肌梗塞、严重的传导阻滞及心功能衰竭等；包括严重肺部感染、严重肺气肿、肺心病及呼吸衰竭等；包括严重脑缺血、严重脑梗塞等；患有其他终末期疾病者；无法接受或配合内镜治疗者； 5）合并有消化性溃疡穿孔，需要外科手术者； 6）严重肝肾功能不全； 7）研究者认为不适合入选者 8）妊娠或哺乳期妇女；

Exclusion criteria：

1)Patients with high suspicion of malignant ulceration; 2)Patients with other tumors; 3)Patients with portal hypertension with varicose vein bleeding 4)Severe cardiopulmonary and cerebrovascular diseases including acute myocardial infarction, severe conduction block and heart failure; Including severe lung infection, severe emphysema, cor pulmonale and respiratory failure; Including severe cerebral ischemia and severe cerebral infarction; Suffering from other end-stage diseases; Unable to accept or cooperate with endoscopic treatment, such as coma; 5)Complicated with peptic ulcer perforation requiring surgery; 6)Severe hepatic and renal insufficiency; 7)Researchers think it is not suitable for the selected candidates; 8)Pregnant or lactating women;
