研究目的：

初步探讨和构建类风湿关节炎慢病管理模式，通过随机平行对照试验设计，比较干预前后对照组和试验组类风湿关节炎患者的生活质量、中医症状积分、患者评估的疼痛程度VAS评分、患者自身对疾病活动的总体评估（PGA）、医生对疾病活动的总体评估（EGA）、关节压痛数、关节肿胀数、血沉、C反应蛋白、临床疾病活动指数（CDAI）、基于28关节疾病活动度评分（DAS28）、用药依从性、复诊率等指标，评价中医慢病管理对类风湿关节炎患者的生活质量、疾病活动度、用药依从性、复诊依从性的影响，为今后类风湿关节炎中医慢病管理模式优化和推广提供参考。  

Objectives of Study：

Preliminary exploration and construction of a chronic disease management model for rheumatoid arthritis using C-reactive protein. A randomized parallel controlled trial design was conducted to compare the quality of life, traditional Chinese medicine symptom scores, pain assessment VAS scores, overall assessment of disease activity by patients themselves (PGA), overall assessment of disease activity by doctors (EGA), joint tenderness count, joint swelling count, erythrocyte sedimentation rate Evaluate the impact of traditional Chinese medicine chronic disease management on the quality of life, disease activity, medication compliance, and follow-up compliance of rheumatoid arthritis patients using indicators such as C-reactive protein, clinical disease activity index (CDAI), 28 joint disease activity score (DAS28), medication compliance, and follow-up rate, providing reference for the optimization and promotion of traditional Chinese medicine chronic disease management mode in rheumatoid arthritis in the future.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）符合2010年ACR/EULAR的RA诊断标准； （2）符合尪痹中医病证诊断疗效标准； （3）年龄 18—70岁，性别不限； （4）愿意配合且能如实完成临床信息调查表； （5）患者了解参与此课题的意义及临床研究中可能出现的风险，自愿加参与该研究，并签署知情同意书。 （6）患者意识清楚并且语言表达清晰，无精神或神经疾病及其他妨碍心理测试的疾病，能够配合完成调查。 （7）患者具有认知能力、识字能力，能自行或在他人帮助下使用智能手机，能积极配合保证治疗的连续性及定期随访。

Inclusion criteria

(1) Meets the 2010 ACR/EULAR diagnostic criteria for RA; (2) Conforming to the diagnostic efficacy criteria for traditional Chinese medicine syndrome of Bi syndrome; (3) Age 18-70 years old, regardless of gender; (4) Willing to cooperate and able to truthfully complete clinical information questionnaires; (5) The patient understands the significance of participating in this topic and the potential risks in clinical research, voluntarily participates in this study, and signs an informed consent form. (6) The patient has a clear consciousness and clear language expression, no mental or neurological diseases and other diseases that hinder psychological test, and can cooperate to complete the investigation. (7) Patients have cognitive and literacy abilities, can use smartphones on their own or with the help of others, and can actively cooperate to ensure the continuity of treatment and regular follow-up.

排除标准：

（1）合并心脑血管、肝、肾、内分泌、造血系统等严重原发性疾病者及重叠其他风湿免疫疾病者：如痛风性关节炎、系统性红斑狼疮、干燥综合征等；
（2）孕妇、哺乳期或有生育要求的患者；
（3）患者精神分裂症、癫痫等严重精神疾病或病情危重，无法配合完成试验者；
（4）近期有手术病史的患者或近期拟行手术者；
（5）有酗酒、吸毒史或神经精神异常病史（癫痫、抑郁症等）；

Exclusion criteria：

(1) Combined with serious primary diseases of the cardiovascular, cerebrovascular, hepatic, renal, endocrine and hematopoietic systems and those with overlapping other rheumatic immune diseases: such as gouty arthritis, systemic lupus erythematosus, dry syndrome, etc;
(2) Patients who are pregnant, lactating or have reproductive requirements;
(3) Patients with serious mental illnesses such as schizophrenia, epilepsy, or critical conditions that prevent them from cooperating to complete the test;
(4) Patients with a recent history of surgery or those who intend to undergo surgery in the near future;
(5) Patients with a history of alcohol or drug abuse or neuropsychiatric abnormalities (epilepsy, depression, etc.)
