研究目的:

本研究通过对解放军总医院第一医学中心住院治疗的高血压患者进行回顾性分析,探究高血压患者中高尿酸血症与心血管疾病的相关性。FRS>10%的患者更易出现心血管疾病,目前高血压合并高尿酸血症患者发生心血管疾病的尿酸cut-off值尚缺乏循证医学证据,本研究通过建立预测模型,探究FRS>10%的高血压发生心血管疾病的尿酸cut-off值,早发现,早治疗,延缓这部分高血压患者心血管疾病的发生以及改善预后。  

Objectives of Study:

This study explored the correlation between hyperuricemia and cardiovascular disease in patients with hypertension through retrospective analysis of the hospitalized patients with hypertension in the First Center of Chinese PLA General Hospital. Patients with FRS>10% are more prone to cardiovascular disease. At present, the uric acid cut-off value of cardiovascular disease in hypertensive patients with hyperuricemia is still lack of evidence-based medical evidence. This study explored the uric acid cut-off value of cardiovascular disease in hypertensive patients with FRS>10% by establishing a prediction model. Early detection and early treatment can delay the occurrence of cardiovascular disease and improve the prognosis.

纳入标准:

第一部分 高血压患者高尿酸血症与CVD的相关性
①2016年1月-2020年12月于我院出院,年龄≥18岁的原发性高血压患者②已完善血尿酸检测
第二部分 FRS>10%的高血压患者CVD预测模型的构建及验证
①2016年1月-2017年12月于我院出院,年龄≥18岁的原发性高血压患者②已完善血尿酸检测③FRS危险评分>10%

Inclusion criteria

Part I Correlation between hyperuricemia and CVD in hypertensive patients

① From January 2016 to December 2020, discharged from our hospital, patients with essential hypertension ≥ 18 years old ② have completed blood uric acid detection

Part II Construction and validation of CVD prediction model for hypertensive patients with FRS>10%

① From January 2016 to December 2017, discharged from our hospital, patients with essential hypertension ≥ 18 years old ② have completed blood uric acid test ③ FRS risk score>10%

排除标准:

第一部分 高血压患者高尿酸血症与CVD的相关性 ①明确的继发性高血压②先天性心脏病③风湿性心脏病④心肌病⑤恶性心律失常⑥严重肝功能不全(ALT和AST>正常上限的3倍)⑦慢性消耗性疾病(严重感染、恶性肿瘤)⑧中度或重度主动脉瓣或二尖瓣返流⑨自身免疫性疾病⑩严重肺部疾病 妊娠妇女 第二部分 FRS>10%的高血压患者CVD预测模型的构建及验证 ①明确的继发性高血压,已发生脑出血、脑梗死、TIA、冠心病、外周动脉疾病、心力衰竭等心血管疾病②先天性心脏病③风湿性心脏病④心肌病⑤恶性心律失常⑥严重肝功能不全(ALT和AST>正常上限的3倍)⑦慢性消耗性疾病(严重感染、恶性肿瘤)⑧中度或重度主动脉瓣或二尖瓣返流⑨自身免疫性疾病⑩严重肺部疾病 11.妊娠妇女

Exclusion criteria:

Part I Correlation between hyperuricemia and CVD in hypertensive patients ① Clear secondary hypertension ② congenital heart disease ③ rheumatic heart disease ④ cardiomyopathy ⑤ malignant arrhythmia ⑥ severe hepatic insufficiency (ALT and AST>3 times the upper limit of normal) ⑦ chronic consumptive disease (serious infection, malignant tumor) ⑧ moderate or severe aortic or mitral regurgitation ⑨ autoimmune disease ⑩ severe lung disease ? pregnant women Part II Construction and validation of CVD prediction model for hypertensive patients with FRS>10% ① Clear secondary hypertension, cerebral hemorrhage, cerebral infarction, TIA, coronary heart disease, peripheral arterial disease Cardiovascular diseases such as heart failure ② congenital heart disease ③ rheumatic heart disease ④ cardiomyopathy ⑤ malignant arrhythmia ⑥ severe hepatic insufficiency (ALT and AST>3 times the upper limit of normal) ⑦ chronic consumptive diseases (severe infection, malignant tumor) ⑧ moderate or severe aortic or mitral regurgitation ⑨ autoimmune diseases ⑩ severe pulmonary diseases 11.pregnant women
