研究目的：

主要研究目的：1.观察术前身体活动强度、血清CLU和GPLD1浓度、自主神经功能评分及心率变异度（HRV）对老年骨科手术患者术后认知功能的影响；2.观察身体活动程度与血清CLU、GPLD1浓度、自主神经功能评分及心率变异度（HRV）、血清炎症因子水平的相关性，对其作用机制进行初步探索。 次要研究目的：1.观察术前身体活动强度、血清CLU和GPLD1浓度、自主神经功能评分及心率变异度（HRV）对老年骨科手术患者术后谵妄及谵妄严重的影响；2.观察术前身体活动强度、血清CLU和GPLD1浓度、自主神经功能评分及心率变异度（HRV）对老年骨科手术患者术后肺部并发症、住院时长、深静脉血栓、伤口感染等预后的影响。  

Objectives of Study：

Main Objectives of the research: 1. To observe the effects of preoperative physical activity intensity, serum CLU and GPLD1 concentrations, autonomic nervous function score and heart rate variability (HRV) on postoperative cognitive function in elderly patients undergoing orthopedic surgery; 2. To observe the correlation between the degree of physical activity and serum CLU and GPLD1 concentrations, autonomic nervous function score, heart rate variability (HRV), and serum inflammatory factors, and to explore its mechanism. Secondary Objectives of the research: 1.To observe the effects of preoperative physical activity intensity, serum CLU and GPLD1 concentrations, autonomic nervous function score and heart rate variability (HRV) on postoperative delirium and its severity in elderly patients undergoing orthopedic surger; 2. To observe the effects of preoperative physical activity intensity, serum CLU and GPLD1 concentrations, autonomic nervous function score and heart rate variability (HRV) on the prognosis of postoperative pulmonary complications, length of hospital stay, deep venous thrombosis and wound infection in elderly patients undergoing orthopedic surgery.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）择期复合麻醉下行骨科手术；
（2）年龄≥60岁且≤90岁，性别不限；
（3）ASA分级在 Ⅰ-Ⅲ级；
（4）BMI 18~30 kg/m2；
（5）可以正常沟通交流；
（6）病人及家属知情同意。

Inclusion criteria

(1) Elective orthopedic surgery under combined anesthesia;
(2) age ≥60 years old and ≤90 years old, both sexes;
(3) ASA grade Ⅰ-Ⅲ;
(4) BMI 18-30 kg/m2;
(5) Communication is normal;
(6) Informed consent of patients and their families.

排除标准：

（1）通过简易精神状态（MMSE）检查小于24分证明存在认知功能损害者； （2）有脑部疾病者，或有脑部疾病病史（有明确临床诊断患者）； （3）存在神经和心理疾病的病史包括AD，中风，精神病等； （4）长期服用镇静类、精神类药物、抗胆碱能药物、抗炎药物的患者； （5）严重的听力或者视力损害； （6）术前收缩压>190mmhg，或舒张压>100mmhg。

Exclusion criteria：

(1) The patients with cognitive impairment by mini-mental State examination (MMSE) score less than 24; (2) The patients with brain disease or a history of brain disease(Patients with a definite clinical diagnosis); (3) The patients had a history of neurological and psychological diseases including AD, stroke, psychosis, etc. (4) The patients with long-term use of sedatives, psychotropic drugs, anticholinergic drugs, and anti-inflammatory drugs; (5) The patients with severe hearing or vision impairment; (6) The patients had preoperative systolic blood pressure >190mmhg, or diastolic blood pressure >100mmhg.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

研究者根据受试者的原始观察记录，将数据原始、完整、正确地载入病例报告表，录入采用相应的数控库系统双人双机录入，之后对数据库进行两遍比对。电子数据文件分类保存，并有多个备份保存于不同磁盘，妥善保存，防止损坏。
