研究目的：

1. 主要目的：通过开展前瞻性干预性临床试验，探究伏隔核/内囊前肢DBS治疗ASD的安全性和有效性，为临床工作提供可靠的参考依据。2. 次要目的：结合影像和神经电生理，阐述DBS治疗ASD的潜在机制。本研究采集研究对象的神经电生理信号，通过数据处理，分析电生理信号与疗效的相关性，寻找潜在的生物标记物。通过影像分析，探究靶点或白质纤维束与临床结果的相关性。  

Objectives of Study：

1.Primary objective: to explore the safety and efficacy of nucleus accumbens/anterior internal capsule DBS in the treatment of autism spectrum disorders (ASD), we perform a prospective intervention clinical study to provide reliable reference for clinical work. 2.Secondary objective: the study explore the potential mechanism of DBS in the treatment of ASD by combining imaging and neuroelectrophysiology. The electrophysiological signals of the patients are collected, and the correlation between the electrophysiological signals and efficacy is analyzed through data processing, so as to find potential biomarkers. We use imaging analysis to explore the correlation between target sites or white matter fiber tracts and clinical outcomes.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1)受试者及监护人应在开始任何与研究相关的程序之前签署研究知情同意书；
2)年龄14-55岁，性别不限；
3)符合《精神障碍诊断和统计手册–第五版》（DSM-V）中关于孤独症谱系障碍(ASD)的诊断标准；
4)经正规治疗（行为治疗、药物治疗、经颅磁刺激等）6个月或更长时间后，ASD的冲动攻击、刻板行为仍未缓解；
5)在整个试验过程中，有生育能力的女性受试者必须使用医学上可接受的避孕方法；
6)患者或监护人必须可以进行充分交流，并且愿意与研究方案充分合作，可以配合完成研究要求的检查、随访及相关的协议。

Inclusion criteria

1)Patients and guardians should sign study informed consent prior to commencement of study-related procedures;
2)The age range from 14 to 55 years old and both sexes are acceptable;
3)The diagnostic criteria conform to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-V);
4)After 6 months or more of formal treatment (behavioral therapy, medication, transcranial magnetic stimulation, etc.), aggression or stereotyped behavior of ASD doesn't reach remission;
5)Fertile female patients must use a medically acceptable method of contraception throughout the trial;
6)Patients or guardians must be able to communicate fully and be willing to cooperate fully with the study protocol and can complete the required tests, follow-up visits, and related protocols.

排除标准：

1)受试者不能达到孤独症谱系障碍的DSM-V诊断标准；
2)严重的人格障碍；认知功能障碍；
3)受试者在筛查前6个月内不能诊断为惊恐障碍、创伤后应激障碍、进食障碍或物质使用障碍；在筛查之前的12个月内不能满足DSM-IV对任何物质依赖性的诊断标准；
4)具有手术禁忌症的受试者，如重大医学合并症或无法停用抗凝药物；
5)术前头部磁共振成像（MRI）显示的任何结构上明显的异常；
6)MRI或PET检查禁忌（除外DBS植入物或刺激器本身）；
7)怀孕；
8)受试者不能配合完成研究要求的测试和随访等，正在参加与当前研究无关的另一项研究。
9）患有重大躯体疾病。

Exclusion criteria：

1)The diagnostic criteria of the patients doesn't conform to DSM-V for autism spectrum disorder;
2)Patients suffer severe personality disorder or cognitive dysfunction;
3)Patients suffer comorbid diseases within 6 months prior to screening including panic disorder, post-traumatic stress disorder, eating disorder, or substance use disorder; Patients suffer substance dependence, which conform to the diagnostic criteria of DSM-V, in the 12 months prior to screening;
4)Patients have contraindications of surgery, such as major medical comorbidities or inability to discontinue anticoagulants;
5)Preoperative magnetic resonance imaging (MRI) of the head reveals structurally significant abnormality;
6)Patients have contraindications of MRI or PET (except for DBS implants or the stimulator itself);
7)Pregnancy;
8)The patients are unable to cooperate to complete the required tests and follow-up and so on;Patients are participating in another study unrelated to the current study;
9)Patients suffer from severe physical disease.
