研究目的：

本研究拟在长期护理保险政策下探索脑卒中患者延续护理模式方案的制订，以促进患者功能的最大恢复和生活质量的提高，同时推动长期护理保险制度的落实，亦为优化照护服务资源配置提供重要依据。  

Objectives of Study：

This study intends to explore the formulation of a continuation care model for stroke patients under the long-term care insurance policy, so as to promote the maximum recovery of patients' function and improve the quality of life, and promote the implementation of the long-term care insurance policy, and also provide an important basis for optimizing the allocation of nursing service resources.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）符合《中国脑血管病防治指南》中脑卒中（包括脑梗死、脑出血、蛛网膜下腔出血）的诊断标准，并经CT/MR确诊；
（2）年龄≥60岁;
（3）mRS≥2分;
（4）广州职工医保;
（5）知情同意并签署知情同意书。

Inclusion criteria

(1) Meet the diagnostic criteria for stroke (including cerebral infarction, cerebral hemorrhage, and subarachnoid hemorrhage) in the "Chinese Guidelines for the Prevention and Treatment of Cerebrovascular Disease", and be diagnosed by CT/MR;
(2) Age ≥ 60 years old;
(3) mRS≥ 2 points;
(4) Guangzhou employee medical insurance;
(5) Informed consent and sign the informed consent form.

排除标准：

（1）患有严重的急性疾病或处于慢性疾病急性发作期者；
（2）心、肝、肾等严重器官功能障碍及恶性肿瘤患者。

Exclusion criteria：

(1) Those who have serious acute diseases or are in the acute attack period of chronic diseases;
(2) Patients with serious organ dysfunction and malignant tumors such as heart, liver, and kidney.
