研究目的：

1、探讨rTMS在促进意识障碍（DOC）患者恢复中的作用，明确其调控机制是否通过增加大脑默认网络(DMN)连接增强脑的功能整合，进而恢复DOC的意识水平，为rTMS治疗意识障碍提供依据。 2、探索rTMS对DOC治疗的效果。? 3、探讨DOC后大脑DMN连接与意识行为的相关性。  

Objectives of Study：

1. To explore the role of rTMS in promoting the recovery of patients with disorders of consciousness (DOC), and to clarify whether its regulatory mechanism enhances the functional integration of the brain by increasing the default network (DMN) connections in the brain, thereby restoring the level of consciousness of DOC, and providing rTMS for the treatment of disorders of consciousness. in accordance with. 2. To explore the effect of rTMS on DOC treatment. 3. To explore the correlation between brain DMN connection and conscious behavior after DOC.

纳入标准：

1、符合意识障碍的诊断标准: 昏迷的诊断标准(Plum et a1.，1972):既无睡眠—觉醒周期，也无意识内容，闭眼状态，无自主行为反应。植物状态的诊断标准(A report of a working party of the Royal College of Physiclans，2003):能够觉醒，存在睡眠一觉醒周期，缺乏功能思维活动，如对自身和周围环境不能知晓，不能与他人交流，无倾向性意图，但具备稳定的自主呼吸及循环功能。最小意识障碍诊断标准：出现以下一项或多项条件后明确诊断：(1)简单的指令行为；(2)手势或语言上做出“是／不是”的反应，无论正确与否；(3)理解语言表达；(4)对相关的环境刺激偶尔做出移动或有效的行为，而不是反射性的活动。
2、无精神、心理病史;3、无癫痫病史;4、无认知障碍病史；5、右利手;6、无酗酒史;7、无严重肝、肾疾病病史;8、无严重传染病病史;9、镇静镇痛药品清洗期后。

Inclusion criteria

1. Meet the diagnostic criteria for disturbance of consciousness: The diagnostic criteria for coma (Plum et a1., 1972): neither sleep-wake cycle nor unconscious content, eyes closed, no voluntary behavioral response. The diagnostic criteria for vegetative state (A report of a working party of the Royal College of Physiclans, 2003): able to wake up, with a sleep-wake cycle, lack of functional thinking activities, such as being unable to know oneself and the surrounding environment, unable to communicate with others, No tendentious intentions, but stable spontaneous breathing and circulation. Diagnostic criteria for minimal disturbance of consciousness: The diagnosis is made after one or more of the following conditions: (1) simple commanding behavior; (2) gesture or verbal "yes/no" response, whether correct or not; (3) ) understanding of language expressions; (4) occasional movement or effective behavior in response to relevant environmental stimuli, rather than reflex activities.
2. No history of mental or psychological diseases; 3. No history of epilepsy; 4. No history of cognitive impairment; 5. Right-handedness; 6. No history of alcoholism; 7. No history of serious liver and kidney diseases; 8. No serious infectious diseases Medical history; 9. After the cleaning period of sedative and analgesic drugs.

排除标准：

1、自主呼吸功能不稳定;2、血流动力学不稳定；3、恶性颅内高压;4、严重或者恶性的心律失常；5、既往癫痫发作者；6、扫描过程中头动大于5毫米;7、体内含有金属类植入物;8、大脑皮层受损达到三分之一的患者;9、家属不同意检查者。

Exclusion criteria：

1. Unstable spontaneous respiratory function; 2. Hemodynamic instability; 3. Malignant intracranial hypertension; 4. severe or malignant ventricular arrhythmia; 5 patients with previous epilepsy; 6. Head movement greater than 5 mm during scanning; 7. Metallic implants in the body; 8, patients with up to a third of the cerebral cortex damaged; 9, the family does not agree with the examiner.
