探讨医护访视教育对居家2型糖尿病患者自我管理和糖代谢控制的影响  

Objectives of Study：

To explore the influence of medical visit education on self-management and glycometabolism control in patients that occupy the home with type 2 diabetes

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①符合1999年世界卫生组织（WHO）制定的2型糖尿病诊断标准；②年龄60-75岁；③就医不便，居住地和医院距离超过6公里，就医缺乏家人陪伴；④自愿参加且签署知情同意书。

Inclusion criteria

Meeting the diagnostic criteria for type 2 diabetes established by the World Health Organization (WHO) in 1999; Age 60-75 years old; It is difficult to get medical treatment, the distance between the residence and the hospital is more than 6 kilometers, and the lack of family members to accompany them; Volunteer and sign informed consent

排除标准：

①1型糖尿病患者；②伴有糖尿病严重并发症；③合并严重心、肺及肝等脏器功能不全；④不能坚持配合访视者；⑤正在参加其他临床研究的患者

Exclusion criteria：

① Patients with type 1 diabetes; ② Severe complications of diabetes; ③ Severe heart, lung, liver and other organ insufficiency; (4) Can not adhere to cooperate with the visitors; ⑤ Patients who are participating in other clinical studies
