本研究的目的是：(1)确定在中国初级保健环境中采用和实施糖尿病SMA的潜在障碍；(2)在可预见的实施障碍的条件下，确定最佳的SMA组建，实现最佳健康结果。  

Objectives of Study：

The objectives of this study are: (1) to identify the potential barriers to adopting and implementing SMA for diabetes in the primary care settings of China; (2) to identify the optimal configuration of SMA components that can potentially achieve the best health outcomes under the conditions of tackling foreseeable implementation barriers.

纳入标准：

医生： 参与试验的机构人员包括公卫医生和临床医生。公卫医生接受过公共卫生方面的培训，负责疾病管理，而临床医生则接受过临床培训，侧重于治疗性护理。由于实施能力的考虑，每个机构将指定一名公卫医生和一名临床医生，负责管理和提供患者的临床治疗、参与试点和优化试验。公卫医生负责糖尿病患者的健康管理。临床医生承担糖尿病患者的门诊服务工作。
患者： 目前在抽样机构中参加政府糖尿病管理项目的患者可以参加试点试验。参与者需满足：（1）符合国家II型糖尿病的诊断标准；（2）实际居住在社区；（3）年龄在35岁或以上。(4)具有完全认知和行为能力；(5)最近半年内没有长期外出计划。同一组中的患者，当他们临床症状相似时，可被纳入其中，以减少临床医生和其他参与者所花费的咨询时间和团体访问的复杂性。同时，若患有其他慢性病的糖尿病患者将被排除在试点试验之外。在确定优化试验的排除标准时，将考虑利益相关者的偏好和试验的可行性。
其他利益相关者： 除临床医生和患者外，作为试验的一部分，以下利益相关者将被邀请参与定性研究：政策制定者、主任、其他在基于团队的SMA护理服务中发挥支持作用的临床医生，以及患者的亲属或朋友。

Inclusion criteria

Clinicians: Those centers typically have both public health doctors and clinical doctors. The former are usually trained in public health and are responsible for disease management, while the latter are clinically trained and focus on curative care. Each institution will designate one capable public health doctor and one clinical doctor who were responsible for managing and providing the clinical treatment for the PWD in their everyday work to participate in the pilot and optimization trials due to the implementation ability consideration. The public health doctor is taking responsibility for patients with diabetes health management. The clinical doctor takes the role of outpatient service for patients with diabetes.
Patients: People with diabetes who are currently enrolled in the government's diabetes management program at our sampled institutions will be eligible to participate in our trial. They should (1) meet the national diagnostic criteria for type II diabetes; (2) physically reside in the communities; and (3) be aged 35 years or older. (4)with complete cognitive and behavioral abilities; (5)no long-term outing plan in recent half a year. The patients in the same cluster will be enrolled when they hold similar conditions to reduce the consultation time spent and complexity of group visiting for clinicians and other patient participants. People with diabetes who also have other chronic conditions will be excluded from the pilot trial. When determining the exclusion criteria in the optimization trial, stakeholders' preferences and trial feasibility will be considered.
Other stakeholders: Besides clinicians and patients, the following stakeholders will be invited to participate in a qualitative study as part of the trial: policy-makers, directors, other clinicians who play a supportive role in team-based SMA care delivery, and relatives or friends of the patients.

一名独立于SMART项目数据收集的统计人员将完成干预分配。分配给每个机构的SMA 干预包类型将遵循R软件MOST软件包（第0.1.0版）中的“随机分配生成器（Version 0.1.0）”生成的随机数字表对参与机构进行随机分组。

Randomization Procedure (please state who generates the random number sequence and by what method)：

A statistician independent of the SMART program data collection will complete the intervention assignment. The type of SMA IP assigned to each institution will follow the random number table generated by the "Random Assignment Generator(Version 0.1.0)" in the MOST package (Version 0.1.0) of the R software.
