研究目的：

评价两组局部复发鼻咽癌患者（A：经口机器人手术组，B：经鼻内镜手术组）治疗后的 1 年无局部复发生存率及术后6月患侧鼻腔容积增加率。  

Objectives of Study：

To evaluate the 1-year local recurrence free survival rate and the volume increase rate of affected side nasal cavity 6 months after the treatment of two groups of patients with locally recurrent nasopharyngeal carcinoma (A: oral robotic surgery group, B: endoscopic surgery group).

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）鼻咽病变组织病理学上被诊断为未分化或分化的非角化的局部复发性鼻咽癌;
（2）按照 AJCC 第八版分期，病变位于鼻咽腔（rT1），鼻孔或鼻中隔（rT2a），浅咽旁间隙（rT2b）或蝶窦的底壁（rT3），定义为手术可切除病变；
（3）距离放疗结束至少1年；
（4）卡氏评分（Karnofsky scale, KPS）≥70；
（5）年龄18-70岁，男女不限；
（6）有手术意愿，能够提供知情同意，并参与定期随访；
（7）未经系统性全身治疗，如化疗、免疫治疗等。

Inclusion criteria

(1) The histopathology of nasopharyngeal lesions was diagnosed as undifferentiated or non keratinized locally recurrent nasopharyngeal carcinoma; (2) According to AJCC 8th Edition staging, the lesions are located in the nasopharyngeal cavity (rT1), nostril or nasal septum (rT2a), shallow parapharyngeal space (rT2b) or the bottom wall of sphenoid sinus (rT3), which are defined as surgical resectable lesions; (3) At least 1 year from the end of radiotherapy; (4) Karnofsky scale (KPS) ≥ 70; (5) Age 18-70, male or female; (6) Willing to operate, able to provide informed consent and participate in regular follow-up; (7) Without systemic treatment, such as chemotherapy and immunotherapy.

排除标准：

（1）复发性疾病已接受挽救性治疗；（2）远处转移；（3）超出可切除区域的广泛复发性原发肿瘤，例如颅底的广泛侵犯或从肿瘤边缘到颈内动脉的距离小于0.5cm；（4）不可切除的淋巴结复发，如侵及颈动脉鞘；（5）初程放射毒性达到3级或更差；（6）威胁生命的肝肾、心脏等器官疾病，凝血功能异常或出血倾向；（7）神经系统或精神类疾病；（8）不愿意签署知情同意书或不愿意参与本研究；（9）依从性较差的患者；（10）张口受限的患者；（11）妊娠及哺乳期女性。

Exclusion criteria：

(1) Recurrent disease has received salvage treatment; (2) Distant metastasis; (3) Extensive recurrent primary tumors beyond the resectable area, such as the extensive invasion of the skull base or the distance from the tumor edge to the internal carotid artery is less than 0.5cm; (4) Recurrence of unresectable lymph nodes, such as invasion of carotid sheath; (5) The initial radiation toxicity reaches grade 3 or worse; (6) Life threatening diseases of liver, kidney, heart and other organs, abnormal coagulation function or bleeding tendency; (7) Nervous system or mental diseases; (8) Not willing to sign the informed consent or participate in this study; (9) Patients with poor compliance; (10) Patients with limited mouth opening; (11) Pregnant and lactating women.
