研究目的：

探索出一种适合卵巢储备正常的子宫腺肌病患者IVF的首选治疗方案，以期获得一种高效的、对孕妇及围产儿相对安全的IVF治疗方案，并为子宫腺肌病患者诊疗方案的选择提供循证医学证据。  

Objectives of Study：

To explore a preferred IVF treatment plan for adenomyosis patients with normal ovarian reserve, in order to obtain an efficient and relatively safe IVF treatment plan for pregnant women and perinatal infants,and to provide evidence-based medical evidence for the choice of diagnosis and treatment options for patients with adenomyosis.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）年龄25-40岁；
2）基础窦卵泡≥5个，或AMH≥1.2 ng/L；
3）临床诊断为子宫子宫腺肌病，符合IVF指征，包括输卵管因素，排卵障碍，内异症、3次人工受精失败以及男性因素等；
4）子宫腺肌症的诊断：根据2021子宫腺肌病伴不孕症诊疗中国专家共识，推荐采用2018年国际妇产科协会（International Federation of Gynecology and Obstetrics, FIGO）授权子宫形态超声评价（Morphological Uterus Sonographic Assessment, MUSA）协作组制定的基于TVUS影像诊断子宫腺肌病的标准。共8项超声特征，分别为子宫肌层不对称增厚；子宫肌层囊性灶；岛状高回声信号；扇形阴影；子宫内膜下线状或点状回声；病灶内有条状血流信号穿过；结合带形态不规则；结合带不连续，结合带的评价建议结合3D-TYUS检查来确认。如超声检查存在2项或2项以上上述征象则拟诊子宫腺肌病。

Inclusion criteria

1) Age 25-40 years old;
2) Basal antral follicles≥5, or AMH≥1.2 ng/L;
3) Clinical diagnosis of uterine adenomyosis, in line with IVF indications, including tubal factors, ovulation disorders, endometriosis, 3 failed artificial inseminations, and male factors;
4) Diagnosis of adenomyosis: According to the consensus of Chinese experts on the diagnosis and treatment of adenomyosis with infertility in 2021, it is recommended to use the TVUS imaging-based diagnosis of adenomyosis developed by the Collaborative Group of Morphological Uterus Sonographic Assessment authorized by the International Federation of Gynecology and Obstetrics in 2018. A total of 8 ultrasound features are: asymmetric myometrial thickening; myometrial cystic foci; island hyperechoic signal; fan-shaped shadow; subendometrial linear or punctate echo; there are strips of blood flow signals passing through the lesion; the binding band is irregular in shape; the binding band is discontinuous, and the evaluation of the binding band is recommended to be confirmed by 3D-TYUS examination. If two or more of the above-mentioned signs are present on ultrasonography, adenomyosis is suspected.

排除标准：

1）全身的系统性疾病；
2）植入前胚胎遗传学诊断周期，捐卵周期，冻卵周期，供精周期；
3）高泌乳素血症，卵巢肿瘤，子宫畸形（双子宫，纵膈子宫等）；
4）输卵管积水或内膜息肉未处理的患者；
5）急慢性生殖道肿瘤、生殖道炎症及并发其它不适宜促排卵疾病的患者；
6）近3个月内服用激素类药物治疗的患者。

Exclusion criteria：

1) Systemic diseases of the whole body;
2) Preimplantation embryo genetic diagnosis cycle, egg donation cycle, egg freezing cycle, sperm donation cycle;
3) Hyperprolactinemia, ovarian tumors, uterine malformations (double uterus, septate uterus, etc.);
4) Patients with untreated hydrosalpinx or endometrial polyps;
5) Patients with acute and chronic genital tract tumors, genital tract inflammation and other diseases that are not suitable for ovulation induction;
6) Patients who have taken hormone drugs within the past 3 months.
