研究目的：

目前，肿瘤切除、淋巴结清扫联合皮瓣转移是治疗口腔癌最有效的手术方式。但是文献报道皮瓣移植术后的坏死率高达10～15%，严重影响了口腔癌患者术后的转归。剧烈的氧化应激和炎症反应，引起血管内皮损伤，促进白细胞及血小板聚集，血栓形成、血管脆性增加等因素可能是导致皮瓣移植术后组织血运障碍、水肿及坏死的潜在机制。围术期促进血管新生，改善局部血液循环、抑制炎症介质的产生、减轻缺血再灌注损伤、抑制皮瓣组织细胞的氧化应激等是防治皮瓣坏死的关键。研究表明，右美托咪定可通过减轻氧化应激和炎症损伤、促进血管生成、稳定血流动力学等途径提高大鼠背侧缺血皮瓣的存活。但术中使用右美托咪定是否可以减轻手术应激和炎症反应，改善口腔癌皮瓣重建术患者术后恢复质量以及减少术后皮瓣并发症的发生尚不清楚，因此本研究旨在探究右美托咪定对口腔癌皮瓣重建术患者术后恢复质量及皮瓣存活的影响，为口腔癌皮瓣重建术患者术中麻醉用药提供参考依据。  

Objectives of Study：

At present, tumor resection, lymph node dissection combined with flap transfer are the most effective surgical methods for the treatment of oral cancer. However, it has been reported in the literature that the necrosis rate after flap transplantation is as high as 10 to 15%, which seriously affects the postoperative outcome of patients with oral cancer. Severe oxidative stress and inflammatory reaction, causing vascular endothelial injury, promoting leukocyte and platelet aggregation, thrombosis, increased vascular fragility and other factors may be the potential mechanism leading to tissue blood circulation disorders, edema and necrosis after flap transplantation. Perioperative promoting angiogenesis, improving local blood circulation, inhibiting the production of inflammatory mediators, reducing ischemia-reperfusion injury, and inhibiting oxidative stress of flap tissue cells are the key to prevent and treat flap necrosis. Studies have shown that dexmedetomidine can improve the survival of dorsal ischemic flaps in rats by alleviating oxidative stress and inflammatory injury, promoting angiogenesis, and stabilizing hemodynamics. However, whether intraoperative use of dexmedetomidine can reduce surgical stress and inflammatory response, improve the quality of postoperative recovery in patients undergoing oral cancer flap reconstruction and reduce the occurrence of postoperative flap complications is still unclear, so the aim of this study was to investigate the effect of dexmedetomidine on the quality of postoperative recovery and flap survival in patients undergoing oral cancer flap reconstruction and provide a reference basis for intraoperative anesthetic medication in patients undergoing oral cancer flap reconstruction.

纳入标准：

1．口腔癌皮瓣重建术患者； 2．年龄≥18周岁； 3．ASA分级Ⅰ～Ⅲ级； 4．本人同意并签署临床研究知情同意书。

Inclusion criteria

1. Patients with oral cancer flap reconstruction

2. Age ≥ 18 years old

3. ASA grade I-III
4. I agree and sign the informed consent form for clinical research.

排除标准：

1．体质量指数（BMI）≥ 35 ㎏/m2； 2．ASA分级Ⅳ级以上； 3．长期服用糖皮质激素者； 4．麻醉药过敏者； 5．精神神经疾病者。

Exclusion criteria：

1. body mass index (BMI) ≥ 35 kg/m2

2. ASA grade Ⅳ or higher

3. long-term use of glucocorticoids

4. narcotic allergy
