研究目的：

1、通过高血压慢病管理APP的应用，评价其在高血压慢病管理中的作用。 2、通过高血压慢病管理APP，同步记录患者治疗方案的使用状况，并引入既往临床研究安全有效的中医降压ABC方案，由患者自主选择练习，探究慢病管理APP在高血压慢病管理上起到的作用。  

Objectives of Study：

1. Evaluate its role in the management of chronic hypertension disease through the application of APP. 2. The application status of patients' treatment plan was recorded synchronously through the APP for chronic hypertension management, and ABC program for lowering blood pressure of traditional Chinese medicine, which was safe and effective in previous clinical studies, was introduced. Patients were allowed to choose exercises independently to explore the role of APP for chronic hypertension management in chronic hypertension management.

纳入标准：

1.年龄18-65岁，男女不限；
2.符合高血压诊断。

Inclusion criteria

1. Male and female aged 18-65.
2. Meet the diagnosis of hypertension.

排除标准：

1.不具备APP安装条件或不能自主使用APP进行自我管理的患者；
2.不能规律检测自身血压者；
3.严重精神病患者、恶性肿瘤等重大疾病患者；
4.研究者认为不适合参加此研究者。

Exclusion criteria：

1. Patients who do not meet the requirements for APP installation or cannot use APP for self-management.
2. Patients who can't check their own blood pressure regularly.
3. Patients with severe mental illness, malignant tumor and other serious diseases;
4. The researcher thinks it is not suitable to participate in this study.
