研究目的:

本课题拟开展基于闭环控制的空间阵列相干电治疗脊髓损伤临床策略研究,制定视觉和听觉诱发、表面肌电为刺激阈值、激活运动功能为刺激目的、调整刺激参数为反馈结果的闭环电刺激治疗策略。在此基础上,利用脊髓损伤有限元模型,设计激活脊髓运动通路的相干电空间阵列方案。最后,利用核磁、电生理、血液生化指标和功能评分等指标进行多层次临床治疗效果评价,为脊髓损伤临床治疗提供科学依据和技术支持。  

Objectives of Study:

In this project, it is proposed to carry out research on clinical strategies of spatial array coherent electrotherapy for spinal cord injury based on closed-loop control, and formulate closed-loop electrical stimulation treatment strategies with visual and auditory induction, surface electromyography as the stimulation threshold, motor function activation as the stimulation purpose, and adjustment of stimulation parameters as feedback results. On this basis, the finite element model of spinal cord injury is used to design a coherent electric spatial array scheme that activates the spinal cord motor pathway. Finally, multi-level clinical treatment effect evaluation is carried out by using nuclear magnetism, electrophysiology, blood biochemical indexes and functional scores, which provides scientific basis and technical support for the clinical treatment of spinal cord injury.

纳入标准:

1)病程1-6个月;2)经患者病史和CT以及MRI诊断确诊为不完全性脊髓损伤,符合美国脊柱损伤协会(ASIA)分类标准中的B-D级,伴有下肢功能障碍;3)损伤患者已经过相关诊疗,现今各项生命体征平稳并进行康复训练的患者;4)患者或患者家属清楚实验全部流程,自愿签署知情同意书。

Inclusion criteria

1) The course of the disease is 1-6 months; 2) Incomplete spinal cord injury confirmed by patient history and CT and MRI diagnosis, meeting the B-D level in the American Association for Spinal Injury (ASIA) classification standards, accompanied by lower extremity dysfunction; 3) Patients with injuries who have undergone relevant diagnosis and treatment, and whose vital signs are stable and undergoing rehabilitation training; 4) The patient or patient's family is clear about the entire process of the experiment and voluntarily signs the informed consent form.

排除标准:

1)合并严重脑外伤、胸腹腔损伤、周围神经损伤、四肢骨折、关节周围异位骨化等情况影响影响神经功能检查者;
2)生命体征不稳、患有精神疾病或意识障碍者;
3)合并心、肝、肾和造血系统等严重原发病者;
4)存在脊髓炎、多发性硬化、脊髓血管畸形致脊髓出血,椎管内肿瘤所致脊髓损伤者。

Exclusion criteria:

1) Severe brain trauma, chest and abdominal cavity injury, peripheral nerve injury, limb fracture, ectopic ossification around the joint, etc., affect the neurological function test;
2) People with unstable vital signs, mental illness or impaired consciousness;
3) Patients with serious primary diseases such as heart, liver, kidney and hematopoietic system;
4) Patients with spinal cord hemorrhage caused by myelitis, multiple sclerosis, spinal cord vascular malformation, spinal cord injury caused by spinal canal tumors.
