研究目的：

本研究前瞻性地评估曲安奈德加硫糖铝凝胶填充在预防食管大面积ESD术后狭窄中的作用，旨在开发一种不良反应少的预防手段，对于降低严重狭窄发生率，减轻患者痛苦具有重要意义。  

Objectives of Study：

This study prospectively evaluated the effect of triamcinolone acetonide plus sucralfate gel filling in the prevention of esophageal stricture after large-area ESD, aiming to develop a preventive method with less adverse effect, which is of great significance in reducing the incidence of severe stricture and alleviating the pain of patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①内镜检查提示食管黏膜病变，电子染色下病灶呈褐色改变，边界清晰；放大内镜观察下符合早期食管鳞状细胞癌或高级别上皮内瘤变的内镜诊断标准；②超声内镜提示病灶呈低回声改变，局限于黏膜层内，黏膜下层连续完整；③病变范围累及食管环周3/4及以上；④食管增强CT未见肿瘤呈浸润性生长，未见周围淋巴结累及；⑤活检病理提示为高级别上皮内瘤变或黏膜内癌，符合内镜ESD治疗指征者。

Inclusion criteria

① Endoscopic examination indicated esophageal mucosal lesions, which were brown and had clear boundaries under electron staining. The endoscopic diagnosis criteria of early esophageal squamous cell carcinoma or high-grade intraepithelial neoplasia were met under magnification endoscopy. ② Endoscopic ultrasound suggested that the lesion was hypoechoic, confined to the mucosa, and the submucosa was continuous and complete. ③ The lesions involved 3/4 or more of the circumferential esophagus; ④ Enhanced esophageal CT showed no invasive tumor growth and no involvement of peripheral lymph nodes. ⑤ Patients with high-grade intraepithelial neoplasia or intramucosal carcinoma indicated by biopsy pathology, which met the indications of endoscopic ESD treatment.

排除标准：

①明确的进展期食管癌患者；②超声内镜提示病变累及黏膜下层甚至更深者，不符合ESD治疗指征者；③食管增强CT提示周围淋巴结侵犯，存在远处转移者；④严重心肺疾病、严重的肝肾疾病、严重的血液系统疾病；⑤凝血功能障碍者。

Exclusion criteria：

① Definite advanced esophageal cancer patients; ② Endoscopic ultrasound suggested that the lesions involved submucosa or even deeper, and did not conform to ESD treatment indications; ③ Enhanced esophageal CT indicated peripheral lymph node invasion and distant metastasis. ④ Serious heart and lung disease, serious liver and kidney disease, serious blood system disease; ⑤ Patients with coagulation dysfunction.
