主要研究目的：探究透析中卧位体操训练对维持性血液透析患者认知功能的影响，分析透析中运动对不同年龄段及不同的认知域的改善作用。 次要研究目的：探究透析中卧位体操训练对患者日常生活活动能力、心理功能、生存质量及躯体功能的影响，分析认知功能变化和以上功能状况的相互作用关系。  

Objectives of Study：

Main research purposes: To explore the influence of recumbent gymnastics training on cognitive function of maintenance hemodialysis patients, and analyze the improvement effect of exercise on different age groups and different cognitive domains in dialysis. Secondary research purposes: to explore the influence of recumbent gymnastics training on daily living ability, psychological function, quality of life and physical function of patients in dialysis, and analyze the interaction between cognitive function changes and the above functional conditions.

药物成份或治疗方案详述：

透析中运动组：在常规血液透析治疗及护理的基础上，进行透析时卧位体操，每周3次，为期12周。运动前对患者生命体征及肢体情况进行评估，判定患者是否适合运动。运动中及运动后监测患者有无胸闷、头晕、肌肉关节疼痛等不良反应，如有不良反应及时停止运动。运动分热身活动、正式运动和放松活动三个阶段。热身活动的方式为关节伸展，放松活动的方式为按摩、牵伸或拍打肌肉等。热身活动和放松活动的时长分别约5～10min，正式运动时长约30～40min，总时长约为60min。患者平卧位，弹力带绕过床尾固定于双侧足踝，运动时机为每次透析时上机后前1～2h。指导患者进行透析中卧位体操，共分为四节，分别为屈膝抗阻运动；抗阻直腿抬高运动；髋外展抗阻运动；髋关节屈伸抗阻运动。患者可根据自己的耐受情况调整肢体运动幅度和运动次数。运动强度的评估以Borg主观疲劳感觉评分(ratingsofperceivedexertion，RPE)为准，运动应达中等强度，11～13分。运动中每20min、运动后5min、10min和患者自诉不适时及时监测患者的生命体征及运动肢体情况，对易发生低血糖患者加强血糖监测。 对照组：对患者进行常规血液透析治疗及血液透析常规护理。透析治疗时,患者除必要的生理活动,多数时间平卧休息。无额外运动干预措施。 

Description for medicine or protocol of treatment in detail：

The exercise group in dialysis: on the basis of routine hemodialysis treatment and nursing, they performed lying position gymnastics during dialysis, three times a week, for 12 weeks. Before exercise, evaluate the patient's vital signs and limb conditions to determine whether the patient is suitable for exercise. During and after exercise, monitor the patient for chest tightness, dizziness, muscle and joint pain and other adverse reactions. If there are adverse reactions, stop exercise in time. Sports can be divided into three stages: warm-up, formal sports and relaxation activities. The way of warming up activities is joint extension, and the way of relaxing activities is massage, stretching or beating muscles. The duration of warm-up and relaxation activities is about 5-10 minutes respectively, and the duration of formal exercise is about 30-40 minutes, with a total duration of about 60 minutes. The patient is in the supine position, the elastic band is fixed on both ankles by bypassing the bed tail, and the exercise time is 1~2 hours before the patient gets on the machine during each dialysis. Instruct patients to perform lying position gymnastics during dialysis, which is divided into four sections: knee flexion resistance exercise; Resistance straight leg lifting movement; Hip abduction resistance movement; Hip joint flexion extension resistance movement. Patients can adjust the amplitude and frequency of limb movements according to their tolerance. The evaluation of exercise intensity shall be based on Borg's rating of perceived exertion (RPE), and the exercise intensity shall be moderate, 11~13 points. Monitor the vital signs and moving limbs of patients every 20 minutes during exercise, 5 minutes and 10 minutes after exercise, and patients' self complaints are not timely, and strengthen blood glucose monitoring for patients prone to hypoglycemia. The control group: received routine hemodialysis treatment and routine hemodialysis nursing care. During dialysis treatment, patients lie flat most of the time except for necessary physiological activities. No additional exercise interventions. 

纳入标准：

①接受维持性血液透析的患者；
②年龄≥18 岁；
③规律血液透析治疗 ≥3个月，透析治疗频率为3次/周；
④肌力≥3级；
⑤生命体征稳定，精神、智力正常，意识清楚，无沟通障碍；
⑥自愿参与试验并签署知情同意书；
⑦接受随机化分组。

Inclusion criteria

1) patients receiving maintenance hemodialysis; 2) Age ≥ 18 years; 3) regular hemodialysis treatment ≥ 3 months; the frequency of dialysis treatment is 3 times/week; 4) Muscle strength ≥ grade 3; 5) Stable vital signs, no severe mental disorders or abnormal intelligence, clear consciousness, no communication obstacle; 6) Volunteer to participate in the test and sign the informed consent form; 7) Accept randomized grouping.

排除标准：

①血压异常：严重的高血压（血压超过 180/110mmHg），或低血压（低于90/60mmHg）；
②心肺疾病：严重的心力衰竭、心律失常，不稳定性心绞痛，重度心包积液、瓣膜狭窄，肥厚性心肌病，主动脉夹层等，未控制的肺动脉高压（肺动脉平均压＞55mmHg）；
③急性临床事件：急性全身炎症性疾病或发热、心脑血管疾病急性期、创伤性疾病急性期等；
④新发深静脉血栓的症状，如小腿不正常的发红和肿痛时要暂缓或停止运动；
⑤患者不能配合运动等；
⑥下肢关节肌肉疼痛；
⑦股静脉置管者。

Exclusion criteria：

1) Abnormal blood pressure: severe hypertension (blood pressure more than 180/110mmHg), or hypotension (less than 90/60mmHg); 2) Cardiopulmonary diseases: severe heart failure, arrhythmia, unstable angina, severe pericardial effusion, valve stenosis, hypertrophic cardiomyopathy, aortic dissection, etc., uncontrolled pulmonary hypertension (mean pulmonary artery pressure>55 mmHg); 3) Acute clinical events: acute systemic inflammatory disease or fever, acute stage of cardiovascular and cerebrovascular disease, acute stage of traumatic disease, etc; 4) The symptoms of new deep vein thrombosis, such as abnormal redness and swelling of the lower leg, should be suspended or stopped; 5) The patient cannot cooperate with the exercise; 6) Lower limb joint muscle pain; 7) Femoral vein catheterization.
