研究目的：

采用前瞻性、多中心、开放、倾向性匹配对照法比较基于风险评估的儿童ANCA相关性肾炎(AAGN) 分层治疗临床的有效性和安全性，以期为儿童AAGN患者提供更加有效和安全的基于分层治疗的用药选择依据。  

Objectives of Study：

A prospective, multicenter, open, and tendentity-matched control method was used to compare the clinical effectiveness and safety of stratified treatment for children with ANCA-associated glomerulonephritis (AAGN) based on risk assessment, in order to provide more effective and safe basis for drug selection based on stratified treatment for children with AAGN.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）初诊AAGN病例，起病年龄为5-17岁，完善肾活检且在医院正规随访；
2）所有入组患儿均符合以下纳入标准：
①ANCA分类诊断标准： 符合2007年欧洲药品管理局(EMA)分类算法或2012年CHCC定义的患者。临床分类包括：包括括微型多血管炎（microscopic polyangiitis，MPA）、肉芽肿性多血管炎（granulomatosis with polyangiitis，GPA）和嗜酸性肉芽肿性多血管炎（eosinophilic granulomatosis with polyangiitis，EGPA）；
②AAGN诊断标准：ANCA血管炎合并以下任一项者：
(1) 血尿：肉眼血尿或镜下血尿:1周内3次镜下血尿:尿常规中红细胞>3个/高倍视野(HP)或者＞17个/ul（高于正常值，可根据各中心检验科的标准调整）；
(2) 蛋白尿：满足以下任一项者：①1周内3次尿常规蛋白阳性；②24h尿蛋白定量>150 mg或者尿蛋白/肌酐(mg/mg)>0.2；③1周内3次尿微量白蛋白高于正常值；
(3) 肾功能不全：血肌酐上升>基10%或肌酐清除率下降>25%；

Inclusion criteria

1) The initial age of AAGN patients was 5-17 years old. The renal biopsy was completed and followed up in the hospital;

2) All enrolled children met the following inclusion criteria:

① ANCA classification diagnostic criteria: patients who meet the classification algorithm of European Drug Administration (EMA) in 2007 or the definition of CHCC in 2012. Clinical classification includes: including microscopic polyangitis (MPA), granulomatosis with polyangitis (GPA) and eosinophilic granulomatosis with polyangitis (EGPA);

② AAGN diagnostic criteria: ANCA vasculitis with any of the following:

(1) Hematuria: gross hematuria or microscopic hematuria: three times of microscopic hematuria within a week: red blood cells in urine routine>3/high power field (HP) or>17/ul (higher than the normal value, which can be adjusted according to the standards of each central laboratory);

(2) Proteinuria: Those who meet any of the following requirements: ① routine urine protein is positive three times within a week; ② 24h urine protein content>150 mg or urine protein/creatinine (mg/mg)>0.2; ③ Urinary microalbumin was higher than the normal value three times in a week;

(3) Renal insufficiency: blood creatinine increased>basal 10% or creatinine clearance decreased>25%;

排除标准：

1）诊断时依赖透析1月以上者，或肾活检时肾小球球性硬化比例≥75%者；
2）合并严重感染者（如：弥漫性腹膜炎、重症肺炎、蜂窝组织炎、活动性EB病毒感染、活动性巨细胞病毒感染、乙型肝炎病毒感染、结核感染、真菌感染等）及肿瘤患者。
3）合并其他原发性或者继发性肾脏疾病者（如IgA肾病、膜性肾病、抗肾小球基底膜肾炎、多囊肾、肾发育不良、泌尿道畸形等）；
4）家长或患儿拒绝入组者；

Exclusion criteria：

1) The diagnosis depends on dialysis for more than 1 month, or the proportion of glomerular sclerosis at renal biopsy is ≥ 75%;

2) Patients with severe infection (such as diffuse peritonitis, severe pneumonia, cellulitis, active EB virus infection, active cytomegalovirus infection, hepatitis B virus infection, tuberculosis infection, fungal infection, etc.) and tumor patients.

3) Those with other primary or secondary renal diseases (such as IgA nephropathy, membranous nephropathy, anti-glomerular basement membrane nephritis, polycystic kidney disease, renal dysplasia, urinary tract malformation, etc.);

4) Parents or children refuse to join the group;
