研究目的：

本课题试图探究基于NICOM目标导向性液体治疗在老年膝关节手术中的应用，以期以更无创、更便捷、更舒适的监测手段找到合适的补液方案，维持患者围术期血流动力学稳定，降低并发症发生率，提高术后转归质量，从而为临床医生围手术期液体治疗提供一定的指导。  

Objectives of Study：

This project attempts to explore the application of NICOM-based goal-oriented fluid therapy in elderly knee surgery, in order to find a suitable fluid replacement plan with a more non-invasive, convenient and comfortable monitoring method, maintain the hemodynamic stability of patients during perioperative period, reduce the incidence of complications, and improve the quality of postoperative outcome. So as to provide some guidance for clinicians in perioperative fluid treatment.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

椎管内麻醉下膝关节手术的老年（＞60岁）患者,术前 ASA 分级II-III级；BMI 18-26kg/m 2之间；性别不限；患者知情同意，可配合流程实施。

Inclusion criteria

Elderly patients (> 60 years) undergoing knee surgery under spinal anesthesia,Preoperative ASA grade II-III; BMI 18-26kg/m 2; There is no gender limitation; Informed consent of the patient, can cooperate with the implementation of the process.

排除标准：

术前存在心肺功能不全；严重肝肾功能障碍；严重心律失常（如室颤、房颤、频发室早、频发房早等）及心衰者；椎管内麻醉禁忌；服用抗凝药物治疗患者；椎管内麻醉后无法满足麻醉需求转为全身麻醉患者。

Exclusion criteria：

Preoperative cardiopulmonary insufficiency; Severe liver and kidney dysfunction; Severe arrhythmias (such as ventricular fibrillation, atrial fibrillation, frequent ventricular premature, frequent atrial premature, etc.) and heart failure; Contraindications of intraspinal anesthesia; Treating patients with anticoagulant drugs; Patients who could not meet the needs of anesthesia after spinal anesthesia were transferred to general anesthesia.
