研究目的:

评估强制空气加温预保温对老年脊柱手术患者术中低体温发生率、寒颤、苏醒延迟等不良事件及围术期热舒适度的影响;确定不同强制空气加温温度下预防老年患者术中低体温发生的最佳预保温时长,为临床老年脊柱手术患者术中体低温的预防及管理提供科学的参考依据。  

Objectives of Study:

To evaluate the effects of forced air heating and preheating on the incidence of intraoperative hypothermia, chills, delayed recovery and perioperative thermal comfort in elderly patients undergoing spinal surgery. To determine the optimal prethermal duration for preventing intraoperative hypothermia in elderly patients under different forced air heating temperatures, so as to provide a scientific reference for the prevention and management of intraoperative hypothermia in elderly patients undergoing spinal surgery.

药物成份或治疗方案详述:

手术患者接入麻醉恢复室(PACU)后,立即开始实施强制空气加温预保温,强制空气加温系统为EQUATOR暖风机(型号:EQ-5000),L组设置强制空气加温系统温度为40℃、H组设置强制空气加温系统温度为44℃,对照组仅采用棉毯被动加温。所有研究对象均采用统一材质、大小的棉毯覆盖全身,除了用于动脉导管的手臂和中心静脉导管的颈部。 

Description for medicine or protocol of treatment in detail:

After the patients were connected to the anesthesia recovery room (PACU), the EQUATOR heater (model: EQ-5000) was implemented immediately. The temperature of the forced-air heating system was set at 40℃ in group L and 44℃ in group H, and the control group was passively warmed by cotton blanket only. All subjects were covered with cotton blankets of uniform material and size, except for the arm used for the arterial catheter and the neck for the central venous catheter. 

纳入标准:

1. 年龄≥65岁; 2. 手术方式为后入路腰椎融合术; 3. 手术预计时间2~4h; 4. 全身麻醉; 5. ASA分级Ⅰ~Ⅲ级; 6. 自愿参加本研究并签署知情同意书。

Inclusion criteria

1. Age >= 65 years; 2. Lumbar fusion through posterior approach; 3. Estimated operation time: 2-4 hours; 4. general anesthesia; 5. ASA ⅰ-ⅲ; 6. Voluntarily participate in this study and sign the informed consent.

排除标准:

1. 患者入室体温≥38℃; 2. 鼻咽部异常,无法进行核心体温测量; 3. 原有基础代谢性疾病(如伴甲亢或甲减),术前3d已经存在感染或体温异常; 4. 体温调节异常,如恶性高热及神经安定剂恶性综合征;伴有甲亢及甲减。

Exclusion criteria:

1. The temperature of the patient at home >= 38℃; 2. Abnormal nasopharynx, unable to take core body temperature measurement; 3. Pre-existing underlying metabolic diseases (such as hyperthyroidism or hypothyroidism), infection or abnormal body temperature 3 days before surgery; 4. Abnormal thermoregulation, such as malignant hyperthermia and neuropacifier malignant syndrome; Accompanied by hyperthyroidism and hypothyroidism.
