研究目的：

1.结直肠癌根治术出院后第1 天起口服或通过管饲摄入研究产品至出院后第30 天，评估速熠素对结直肠癌患者术后营养状况、炎症相关实验室指标、免疫学参数、术后开始放化疗治疗时间等方面的疗效情况。 2.同时评估速熠素对能否影响结直肠肿瘤免疫微环境中免疫细胞浸润情况等  

Objectives of Study：

1. From the first day after discharge after radical resection of colorectal cancer to the 30th day after discharge, to evaluate the efficacy of Suonitsu on postoperative nutritional status, inflammation-related laboratory indexes, immunological parameters, and the duration of postoperative radiotherapy and chemotherapy in patients with colorectal cancer. 2. At the same time, to evaluate whether Sumansin can affect immune cell infiltration in the immune microenvironment of colorectal tumors

药物成份或治疗方案详述：

Oral Impact 自上世纪90年代首次上市，已在全球30多个国家和地区上市，进行了60多项临床研究，其中16项为在亚洲展开的研究。受试人员主要为头颈部、胸腹部实体肿瘤围手术期及放化疗患者。这些研究对Impact的安全性、营养充足性、有效性都进行评估，另外有15项meta分析及1篇综述对Impact进行了综合评价。它是目前全球唯一同时含有精氨酸、n-3脂肪酸和核苷酸这三大免疫营养素的专利配方，被国内国外指南广泛推荐。 速熠素是在保留了Oral Impact核心经典配方的基础上，降低糖分提高蛋白质含量，经过三年的随机对照研究正式获批在中国上市的第一款，也是目前唯一一款肿瘤患者专用的营养品（特殊医学用途配方食品）.良好的免疫应答对肿瘤术后康复尤其是伤口愈合至关重要，在肿瘤大手术术后的早期机体会处于一个免疫抑制期 。研究证实患者的免疫功能可在大约术后2周的时间恢复，而这种免疫抑制的状态可增加了肿瘤手术患者发生感染性并发症的几率 ，若再合并营养不良导致细胞复制所需的必需营养素如核苷酸、氨基酸、脂肪酸等营养素缺乏，可进一步削弱免疫系统。围手术期给予免疫营养的概念就是提供必要的营养素，以促进术后充分的免疫反应。研究证实单一免疫营养素的临床获益往往有限，而速熠素特有的三大免疫营养素（精氨酸、n-3不饱和脂肪酸和核苷酸）的临床获益已被超过60项的临床研究证实，被国内国外指南广泛推荐。 研究组：根据术后饮食原则正常进食+肿瘤全营养配方食品（速熠素），速熠素每次1瓶（250ml/瓶），每天2次，相当于每天大约707kcal。速熠素不作为研究组单一营养来源。 对照组：根据术后饮食原则正常进食。 研究时间：胃术后患者出院第1天至第30天。 

Description for medicine or protocol of treatment in detail：

Oral Impact has been available in more than 30 countries and regions since it was first marketed in the 1990s and has conducted more than 60 clinical studies, including 16 in Asia. The subjects were mainly patients with head, neck, chest and abdomen solid tumors in perioperative period and radiotherapy and chemotherapy. These studies evaluated the safety, nutritional adequacy, and effectiveness of Impact. In addition, 15 meta-analyses and 1 review comprehensively evaluated Impact. At present, it is the world's only patented formula containing arginine, n-3 fatty acids and nucleotides at the same time, which is widely recommended by domestic and foreign guidelines. Impact oral is the first nutritional product (formula food for special medical use) for cancer patients that has been officially approved to be marketed in China after three years of randomized controlled study on the basis of retaining the Oral Impact core formula, reducing sugar and improving protein content. Good immune response is very important for postoperative rehabilitation of tumor, especially wound healing. In the early stage after major tumor surgery, the body will be in a period of immunosuppression. Studies have confirmed that the immune function of patients can be recovered in about 2 weeks after surgery, and this immunosuppressed state can increase the risk of infectious complications in patients undergoing tumor surgery. If combined malnutrition leads to the deficiency of essential nutrients for cell replication, such as nucleotides, amino acids, fatty acids and other nutrients, it can further weaken the immune system. The concept of perioperative immunonutrition is to provide the necessary nutrients to promote an adequate postoperative immune response. Studies have proved that the clinical benefits of a single immunonutrient are often limited, while the clinical benefits of the three unique immunonutrients of Almanit (arginine, n-3 unsaturated fatty acids and nucleotides) have been confirmed by more than 60 clinical studies and widely recommended by domestic and foreign guidelines. Study group: According to the dietary principles of Colorectal cancer after operation, normal eating + tumor total nutritional formula food (Impact oral),Impact oral One bottle at a time (250ml/ bottle) twice a day equals approximately 707kcal per day. Impact oral Not as a single source of nutrition for study groups. Control group: Normal diet according to postoperative dietary principles of Colorectal cancer Study period: Patients with Colorectal cancer after surgery were discharged from the 1st day to the 30th day. 

纳入标准：

1.年龄18岁以上，75岁以下，性别不限；
2.经组织学或细胞学诊断确诊为结直肠腺癌的患者，并接受根治性手术的患者；
3.体质指数（BMI）18.5-28kg/m2（含18.5和28kg/m2）；
4.出院后可耐受肠内营养患者；
5.预期寿命超过3个月；
6.出院时血红蛋白≥90g/L；
7.出院时白蛋白≥2.5g/dL；
8.患者能够知情同意，同意参加本研究并签署知情同意书。

Inclusion criteria

1. Age above 18 years old, under 75 years old, regardless of gender;
2. Patients diagnosed with colorectal adenocarcinoma by histology or cytology and undergoing radical surgery;
3. Body mass index (BMI) 18.5-28kg/m2 (including 18.5 and 28kg/m2);
4. Patients who could tolerate enteral nutrition after discharge;
5. Life expectancy over 3 months;
6. Hemoglobin ≥90g/L at discharge;
7. Albumin ≥2.5g/dL at discharge;
8. Patients can give informed consent, agree to participate in the study and sign informed consent.

排除标准：

1.存在严重的临床症状、基础病或手术并发症，如消化道出血、吻合口瘘，可能危及研究的开展及随访；
2.既往胃肠道手术史或其他消化道治疗史并影响营养素吸收；
3.患者为怀孕中或哺乳期的妇女；
4.患者行验证性活检或内窥镜检查等胃肠道肿瘤小手术或行内窥镜下肿瘤切除术；
5.手术前期不可口服或通过管饲摄入营养；
6.患者入组前4周内参加过另一干预性临床研究，包括营养支持相关的研究；
7.过去5年中（除成功治疗的原位基底细胞皮肤肿瘤以及原位宫颈肿瘤）患其他恶性肿瘤；
8.患者存在肝肾功能异常（丙氨酸氨基转移酶ALT≥2倍正常值上限；总胆红素Tbil≥2倍正常值上限；肌酐Cr≥2倍正常值上限）；
9.已知患有未受控制的糖尿病或空腹血糖≥10mmol/L；
10.患者已知有甲状腺功能减退或甲状腺功能亢进病史；
11.患者正在使用含胸腺肽、糖皮质激素、甲状腺素、生长激素、抗肿瘤坏死因子生物制剂等；
12.已知对研究产品的任何成分有过敏史；
13.患者或家属拒绝签署知情同意书；
14.未受控制的心理疾病。
15.研究者认为不适合本次研究的患者（如依从性差等）

Exclusion criteria：

1. There are serious clinical symptoms, underlying diseases or surgical complications, such as gastrointestinal bleeding and anastomotic fistula, which may jeopardize the development and follow-up of the study;
2. Previous history of gastrointestinal surgery or other digestive tract treatments that affect nutrient absorption;
3. The patient is a pregnant or lactating woman;
4. The patients underwent minor operations for gastrointestinal tumors such as confirmatory biopsy or endoscopy, or underwent endoscopic tumor resection;
5. Nutrition should not be ingested orally or through tube feeding before the operation;
6. The patient had participated in another interventional clinical study, including one related to nutritional support, within 4 weeks prior to enrollment;
7. Malignant neoplasms in the past 5 years (other than basal cell skin neoplasms in situ, which were successfully treated, and cervical neoplasms in situ);
8. The patient had abnormal liver and kidney function (ALT≥2 times the upper limit of normal value; Total bilirubin Tbil≥2 times the upper limit of normal value; Creatinine Cr≥2 times the upper limit of normal);
9. Known to have uncontrolled diabetes mellitus or fasting blood glucose ≥10mmol/L;
10. The patient has a known history of hypothyroidism or hyperthyroidism;
11. The patient is using biologic agents containing thymosin, glucocorticoid, thyroxin, growth hormone, anti-tumor necrosis factor, etc.;
12. A known history of allergy to any component of the study product;
13. Patients or family members refuse to sign informed consent;
14. Uncontrolled mental illness.
15. Patients deemed unsuitable for this study by the researcher (such as poor compliance, etc.)
