研究目的：

主要目的：本研究通过前瞻性观察研究的方法，对我院心内科诊断为糖尿病合并心肌梗死的患者，通过出院后是否给予SGLT2抑制剂类药物分为两组，在使用SGLT2抑制剂至出院后两周内进行心脏核磁共振检查作为基础数据，随访时间为3、6、12月，进一步评价心梗标志物及心脏核磁共振数据，为评价这类药物在糖尿病合并心肌梗死患者中的安全性和心脏获益机制提供依据。  

Objectives of Study：

Main purpose: This study, by using the method of prospective observational study in our heart medicine diagnosis of diabetic patients with myocardial infarction (mi), by whether after discharge SGLT2 inhibitor drugs are divided into two groups, in the use of SGLT2 inhibitors to two weeks after discharge inspection as the basic data of cardiac magnetic resonance imaging (mri), patients were followed up for 3, 6, 12 months, To further evaluate myocardial infarction markers and cardiac MRI data, and to provide a basis for evaluating the safety and cardiac benefit mechanism of these drugs in patients with diabetes complicated with myocardial infarction.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①中国男性或女性，年龄≥18 周岁，≤80岁；
②符合2型糖尿病的临床诊断；
③符合急性心肌梗死的临床诊断；
④糖尿病除生活方式干预外，至少需要一种降糖药物治疗（包括磺脲类药物，二甲双胍，阿卡波糖，伏格列波糖，噻唑烷二酮类，格列奈类药物，DPP4抑制剂，GLP-1受体激动剂，胰岛素等），由于急性心梗患者多数合并心衰，因此对于急性心梗和心衰禁忌或慎用的药物在治疗期间尽量规避，如噻唑烷二酮类（吡格列酮、罗格列酮），DPP4抑制剂（沙格列汀，阿格列汀），二甲双胍等。
⑤受试者意识清楚，能满足正常的交流；
⑥依从性：具有较好的依从性，能理解和执行医生的医嘱。

Inclusion criteria

① Chinese male or female, age >= 18, <= 80;
② It was consistent with the clinical diagnosis of type 2 diabetes;
③ It conforms to the clinical diagnosis of acute myocardial infarction;
④ In addition to lifestyle intervention, diabetes needs at least one hypoglycemic drug treatment (including sulfonylureas, metformin, acarbose, voglibose, thiazolidinediones, Gllinides, DPP4 inhibitors, GLP-1 receptor agonists, insulin, etc.), because most patients with acute myocardial infarction complicated with heart failure, Therefore, for acute myocardial infarction and heart failure drugs contraindicated or used with caution should be avoided during treatment, such as thiazolidinediones (pioglitazone, rosiglitazone), DPP4 inhibitors (sagagliptin, alagliptin), metformin, etc.
(5) Subjects are conscious and can meet normal communication;
⑥ Compliance: have good compliance, can understand and carry out the doctor's advice.

排除标准：

①肾小球滤过率eGFR ＜45mL/min/1.73m2。

Exclusion criteria：
