研究目的：

1、考察反复输注小剂量(S)-氯胺酮对阿片耐受癌痛患者的镇痛效应及阿片耐受的逆转作用。 2、考察(S)-氯胺酮在阿片耐受癌痛患者中的量效关系以优化治疗方案。 3、检测肿瘤相关免疫指标，考察(S)-氯胺酮对癌症患者肿瘤免疫的影响。  

Objectives of Study：

1. To investigate the analgesic effect of repeated infusion of low-dose (S)-ketamine on cancer pain patients with opioid tolerance and the reversal effect of opioid tolerance. 2. To investigate the dose-response relationship of (S)-ketamine in patients with opioid-resistant cancer pain to optimize the treatment plan. 3. To detect tumor-related immune indexes, and investigate the effect of (S)-ketamine on tumor immunity in cancer patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.每日至少口服60mg吗啡或30mg羟考酮或其他等量阿片类药物至少一周的癌痛患者，或合用两种以上阿片类药物，或两种以上途径使用阿片类药物的剂量经换算≥相当于口服吗啡60mg，且连续使用至少1周的患者
2.VAS评分≥6分或爆发痛≥3次/24h
3.年龄≥18岁且≤70岁，性别不限
4.理解、同意参加本研究并签署知情同意书

Inclusion criteria

1.Patients with cancer pain who take at least 60 mg of morphine or 30 mg of oxycodone or other equivalent opioids per day for at least one week, or use two or more opioids in combination, or the doses of opioids used in two or more ways converted ≥ equivalent Oral morphine 60mg, and continuous use for at least 1 week in patients.
2.VAS score ≥ 6 points or burst pain ≥ 3 times/24h.
3.Age ≥ 18 years old and ≤ 70 years old, gender is not limited.
4.Understand and agree to participate in this study and sign the informed consent.

排除标准：

1.既往有滥用阿片类药物、酗酒史
2.合并有严重心血管疾病、高血压、严重肝肾功能损害患者
3.意识障碍或者伴严重精神、神经系统疾病的患者
4.目前颅内压或眼内压增高
5.妊娠期或哺乳期妇女

Exclusion criteria：

1.A history of opioid abuse and alcohol abuse
2.Patients with severe cardiovascular disease, hypertension, severe liver and kidney damage
3.Patients with impaired consciousness or with severe mental and nervous system diseases
4.Increased intracranial pressure or intraocular pressure
5.Pregnant or lactating women
