研究目的：

1）建立基于无创个体化心脏建模仿真技术精准预测恶性室性心律失常猝死风险的新方法； 2）开展前瞻性队列研究验证该新方法的有效性。  

Objectives of Study：

1 ) To establish a new method for accurately predicting the risk of sudden death of malignant ventricular arrhythmia based on noninvasive individualized cardiac modeling and simulation technology; 2 ) To verify the effectiveness of the new method by conducting a prospective cohort study.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）18周岁 < 年龄 < 80周岁；
2）药物治疗时间 > 3个月；
3）预期生存时间 > 1年；
4）符合SCD一级预防适应症；（2020中国室性心律失常专家共识）。

Inclusion criteria

(1) adults aged 18–80 years;
(2) symptom cannot be relieved despite optimal medical therapy for at least 3 months;
(3) life expectancy of at least 1 year;
(4) meet with the indication of primary prevention for SCD according to 2020 Chinese Expert Consensus on Ventricular Arrhythmia.

排除标准：

1）无法配合随访；
2）心房颤动；
3）肌酐清除率 < 30ml/min；
4）感染性休克；
5）恶性肿瘤晚期；
6）孕妇；
7）心包压塞或大量心包积液。

Exclusion criteria：

(1) unable to cooperate with follow-up; (2) atrial fibrillation; (3) estimated glomerular filtration rate (eGFR) less than 30 mL/min; (4) septic shock; (5) advanced malignant tumor; (6) pregnancy; (7) cardiac tamponade or major haemopericardium.

指标试验（即本研究的待评估诊断试验，无论为方法、生物标志物或设备，均请列出名称）：

基于术前心脏磁共振检查结果，建立病人个体化解剖模型和单细胞电生理模型，采用有限元方法并结合高性能并行计算机来计算病人是否会发生心律失常和心律失常发生的位置。

Index test:

Based on preoperative cardiac magnetic resonance imaging, the patient's individualized anatomical model and single cell electrophysiological model were established. The finite element method and high-performance parallel computer were used to calculate whether the patient would have arrhythmia and its location.

目标人群（可以是某种疾病患者或正常人群，详细描述其疾病特征，注意应纳入符合分布特点的全序列病例，具有良好的代表性）

于北京安贞医院行ICD植入的心源性猝死一级预防患者。 ICD植入的心源性猝死一级预防I类适应症（《2020中国室性心律失常专家共识》）： 缺血性心脏病导致的LVEF≤0.35，心肌梗死后至少30d或血运重建后至少90d，经最佳药物治疗后心功能(NHYA分级)II-III级，预期生存时间>1年； 缺血性心脏病导致的LVEF≤0.30，心肌梗死后至少30d或血运重建后至少90d，经最佳药物治疗后心功能(NHYA分级)I级，预期生存时间>1年； 非缺血性心脏病导致的LVEF≤0.35，经最佳药物治疗后心功能(NHYA分级)II-III级，预期生存时间>1年。
