探讨多领域护理干预对痴呆临床前期患者的认知功能和认知储备的干预效果  

Objectives of Study：

To explore the effect of multi field nursing intervention on cognitive function and cognitive reserve of patients with pre clinical dementia

药物成份或治疗方案详述：

多领域护理干预：认知训练+生活方式+休闲活动 1.制定依据：《WHO认知能力下降和痴呆风险降低指南》《神经指南：认知训练中国专家共识》 方案内容：团队前期的认知训练程序；其他适合的开源认知训练小游戏（绝世高手、果蔬大战、灭鼠专家等）。 目标：鼓励参与者坚持每周进行3次认知训练、每次训练时间30min。 2.制定依据：《WHO认知能力下降和痴呆风险降低指南》 《柳叶刀痴呆预防、干预、护理委员会建议》 方案内容：报告提出的9条针对政策制定者和个人的建议。 目标：控制危险因素的个数（降低一半）和/或水平（降低一个标准差） 3.制定依据：《WHO认知能力下降和痴呆风险降低指南》 《柳叶刀痴呆预防、干预、护理委员会建议》 方案内容：与家人朋友沟通的技巧；介绍团体性活动的类型；孤独感的应对措施，以及提供社区活动的资源信息。 目标：在中年和老年保持积极生活，增加社会接触，强化社会参与，降低孤独感。 

Description for medicine or protocol of treatment in detail：

Multi field nursing intervention: cognitive training+lifestyle+leisure activities 1. Basis for formulation: WHO Guidelines for Cognitive Impairment and Risk Reduction of Dementia; Neuroguidelines: Consensus of Chinese Experts on Cognitive Training Program content: the team's early cognitive training program; Other suitable open-source cognitive training games (peerless experts, fruit and vegetable wars, rat killing experts, etc.). Objective: Encourage participants to carry out cognitive training 3 times a week for 30 minutes each time. 2. Basis for formulation: WHO Guidelines for Cognitive Impairment and Risk Reduction of Dementia, Recommendations of the Lancet Dementia Prevention, Intervention and Nursing Committee Program content: 9 suggestions for policy makers and individuals proposed in the report. Objective: Control the number of risk factors (reduce by half) and/or level (reduce by one standard deviation) 3. Basis for formulation: WHO Guidelines for Cognitive Impairment and Risk Reduction of Dementia, Recommendations of the Lancet Dementia Prevention, Intervention and Nursing Committee Program content: communication skills with family and friends; Introduce the types of group activities; Measures to cope with loneliness, as well as providing resource information for community activities. Objective: To maintain an active life in middle age and old age, increase social contact, strengthen social participation, and reduce loneliness. 

纳入标准：

1.年龄≥45周岁；
2.符合中国痴呆与认知障碍诊疗指南标准中MCI和SCD诊断标准，且具体量性阈值参考如下：参与者或知情者报告的1年内主观认知功能下降；MMSE得分＞24分（小学：＞20分；文盲：＞17分）；MoCA得分＜26分（若教育年限≤12年，总分加1分），ADL得分＜26分；
3.无视听说功能障碍，知情同意并愿意配合本研究者。

Inclusion criteria

1. Age ≥ 45;
2. It conforms to the diagnostic criteria of MCI and SCD in the Chinese Guidelines for the Diagnosis and Treatment of Dementia and Cognitive Impairment, and the specific quantitative threshold is as follows: the subjective cognitive function reported by participants or insiders decreases within one year; MMSE score ＞ 24 (primary school: ＞ 20; illiterate: ＞ 17); MoCA score ＜ 26 points (if the length of education ≤ 12 years, add 1 point to the total score), ADL score ＜ 26 points;
3. Those who have no visual, audio and oral dysfunction, have informed consent and are willing to cooperate with this study.

排除标准：

1.存在严重神经系统疾病，脑卒中、脑梗死等；
2.存在严重的精神障碍者；
3.过去半年内参加任何认知训练

Exclusion criteria：

1. There are serious nervous system diseases, stroke, cerebral infarction, etc;
2. People with serious mental disorders;
3. Participate in any cognitive training in the past six months
