研究目的：

髋关节置换术大多为老年人，拥有2个特点： 第一：基本情况不好，对血流动力学的改变较为敏感。 第二：理想的术后应该时无痛且尽早运动 理想的神经阻滞应该满足以上两点. A:PENG是提出的不影响运动且提供很好镇痛的神经阻滞（阻滞闭孔神经，股神经，副闭孔神经），但是切口无法阻滞到，切口疼痛是术后疼痛的主要来源，所以术后加上切口局部浸润，这样就保证了更全的镇痛。 B:改良髂筋膜阻滞相比较以往髂筋膜阻滞来说，阻滞闭孔神经与股外侧皮神经成功率更高。（所以就阻滞了股神经，闭孔神经，股外侧皮神经）股外侧皮神经是全髋置换切口主要的支配神经。 所以将两者方法对比，A组使用20ml 0.5% 加切口局部浸润, B组使用40ml 0.25%; 比较对全髋置换术术中血流动力学的影响以及术后疼痛与肌力的影响。  

Objectives of Study：

Hip arthroplasty is mostly for the elderly, with two characteristics: First: The basic situation is not good, and it is sensitive to changes in hemodynamics. Second: The ideal post operation should be painless and exercise as soon as possible The ideal nerve block should meet the above two points A: PENG is a nerve block (blocking obturator nerve, femoral nerve, and accessory obturator nerve) that does not affect movement and provides good analgesia, but the incision cannot be blocked. The incision pain is the main source of postoperative pain, so local infiltration of the incision after surgery is added to ensure more complete analgesia. B: Compared with the previous iliac fascia block, the improved iliac fascia block has a higher success rate in blocking obturator nerve and lateral femoral cutaneous nerve. (Therefore, the femoral nerve, obturator nerve, and lateral femoral cutaneous nerve are blocked.) The lateral femoral cutaneous nerve is the main innervating nerve for the total hip replacement incision. Therefore, comparing the two methods, group A uses 20ml 0.5% plus local infiltration of incision, and group B uses 40ml 0.25%; To compare the influence on hemodynamics, postoperative pain and muscle strength during total hip replacement.

年龄≥18岁
无区域麻醉禁忌症

Inclusion criteria

Patients undergoing hip surgery under general anesthesia and tracheal intubation
ASA Classification I-III
Age ≥ 18
No contraindication of regional anesthesia

排除标准：

不能同意本研究
凝血功能障碍
败血症
肝或肾功能衰竭
局部麻醉(LA)过敏
怀孕
摄入阿片类药物。
