研究目的:

本研究通过对失眠症人群进行rTMS干预,采用视觉数字“N-back”实验范式,阿拉伯数字0~9作为视觉刺激材料,在受试者干预前后,佩戴美国EGI 128导脑电设备进行“N-back”任务测试,在任务的同时记录被试者的行为数据与脑电ERP数据。最后进行行为数据和脑电ERP数据统计学检验,比较干预前两组被试者和失眠症组干预前后的行为学正确率、反应时间、事件相关单位的特征波P1、N2、P3的峰值和潜伏期,主要分析受试者额区Fz、中央区Cz、顶区Pz三个电极点ERP平均波幅的差异。进而明确rTMS干预对非器质性失眠人群工作记忆的改善作用,阐述其机制,探讨其对工作记忆的效应,进而为失眠症进行rTMS物理干预治疗提供理论依据和实践依据。从临床的角度来看,TMS为以后治疗其他类型的睡眠障碍提供了一种安全有效的治疗方案;从公共卫生的角度来看,改善当代年轻人失眠状况、优化睡眠结构意义重大;从脑科学研究来看,更能为深入研究脑ERP机制奠定基础,为更好的将rTMS应用于临床其他睡眠障碍做指导。  

Objectives of Study:

Through rTMS intervention for insomniacs, this study adopted the visual number "N-back" experimental paradigm, and the Arabic numeral 0~9 as the visual stimulation material. Before and after the subjects intervened, they wore American EGI 128 EEG equipment for the "N-back" mission test. At the same time, the behaviour data and ERP data of the subjects were recorded. Finally, the statistical test of behavioural data and ERP data was carried out to compare the peak and incubation period of behavioural accuracy, reaction time and event-related units of the first two groups of subjects and insomnia. It mainly analysed the front area Fz, central area Cz and top area of the subjects. The difference in the average amplitude of ERP at the three electrode points of Pz. Then it clarifies the effect of rTMS intervention on the improvement of working memory in non-orological insomniac, expounds its mechanism, discusses its effect on working memory, and then provides a theoretical and practical basis for rTMS physical intervention treatment for insomnia. From a clinical point of view, TMS provides a safe and effective treatment plan for other types of sleep disorders in the future. From the perspective of public health, it is of great significance to improve insomnia and optimise sleep structure in contemporary young people. From the perspective of brain scientific research, it can lay the foundation for in-depth study of brain ERP mechanisms. In order to better apply rTMS to other clinical sleep disorders.

药物成份或治疗方案详述:

本研究采用干预方法为安全无创的经颅磁刺激。刺激频率1Hz,刺激靶点为右侧前额叶背外侧皮层,刺激强度为80%RMT,刺激25s,间歇3s,刺激43个序列,共1075次脉冲,约20min,每天一次,每周五次,共两周。 

Description for medicine or protocol of treatment in detail:

In this study, the intervention method was used for safe and non-invasive transcranial magnetic stimulation. The stimulation frequency is 1Hz, the stimulation target is the dorsal lateral cortex of the right prefrontal lobe, the stimulation intensity is 80% RMT, the stimulation is 25s, the interval is 3s, the stimulation is 43 sequences, a total of 1,075 pulses, about 20 minutes, once a day, every Friday, for two weeks. 

纳入标准:

1.青年国人非器质性失眠症患者纳入标准:
(1)符合国际疾病分类第10版(International Classification of Disease,ICD-10)关于非器质性失眠症的诊断标准;
(2)抱怨入睡困难、保持睡眠困难或早醒至少1个月;
(3)无其他睡眠障碍,如睡眠异常、睡眠过多、睡眠相关运动障碍或其他精神障碍;
(4)爱丁堡利手量表测量的右手性;
(5)性别不限,年龄18岁-40岁且愿意参加本研究,并签署知情同意书。
2.青年国人正常健康对照组纳入标准:
(1)良好的睡眠质量和良好的睡眠开始和/或维持,匹兹堡睡眠质量指数(PSQI)低于5分;
(2)本研究前至少2个星期未服用任何兴奋剂、药物、香烟、酒精或咖啡;
(3)爱丁堡利手量表测量的右手性;
(4)性别不限,年龄在18岁-40岁且愿意参加本研究,并签署知情同意书。

Inclusion criteria

1. Incorporated patients with non-ororal insomnia in young Chinese:
(1) Meet the diagnostic criteria for non-oral insomnia in the 10th edition of the International Classification of Diseases (ICD-10);
(2) Complain about difficulty falling asleep, keeping sleep difficult, or waking up early for at least 1 month;
(3) No other sleep disorders, such as sleep abnormalities, excessive sleep, sleep-related motor disorders or other mental disorders;

(4) Right hand sex measured by the Edinburgh Hand Scale;
(5) Gender-neutral, 18-40 years old and willing to participate in this study and sign an informed consent form.
2. Normal health control groups in young nationals are included in the criteria:
(1) Good sleep quality and good sleep start and/or maintenance, Pittsburgh Sleep Quality Index (PSQI) is less than 5 points;
(2) Did not take any stimulants, drugs, cigarettes, alcohol or coffee for at least two weeks before this study;
(3) Right-handedness measured by the Edinburgh Hand Scale;
(4) Sex-neutral, 18-40 years old and willing to participate in this study and sign informed consent form.

排除标准:

(1)孕期、哺乳期或月经期(女性);
(2)器质性疾病或继发于抑郁或广泛性焦虑的严重精神疾病所致的失眠;
(3)神经系统或其他躯体疾病史,如呼吸、心脏、肾脏、肝脏和内分泌疾病;
(4) 2周内任何可能影响睡眠或脑功能的药物;
(5)成瘾障碍;
(6)心脏有起搏器、体内有金属者。

Exclusion criteria:

(1) Pregnancy, breastfeeding or menstruation (female);
(2) Insomnia caused by organic diseases or serious mental diseases secondary to depression or generalised anxiety;
(3) History of neurological or other somatic diseases, such as respiratory, heart, kidney, liver and endocrine diseases;
(4) Any drug that may affect sleep or brain function within 2 weeks;
(5) Addiction disorder;
(6) People with pacemakers in the heart and metal in the body.
