研究目的：

（1）探究抗菌喷雾预防任何解剖部位淋球菌感染的效果； （2）探究抗菌喷雾预防口咽、生殖器和肛门淋球菌感染的效果； （3）探究抗菌喷雾预防任何解剖部位沙眼衣原体感染感染的效果； （4）探究抗菌喷雾预防口咽、生殖器和肛门沙眼衣原体感染的效果； （5）探究抗菌喷雾预防梅毒感染的效果。  

Objectives of Study：

(1) To investigate the efficacy of antibacterial spray in the prevention of Neisseria gonorrhoeae infection at any site; (2) To investigate the efficacy of antibacterial spray in the prevention of Neisseria gonorrhoeae infection at oropharynx, urethra or anorectum; (3) To investigate the efficacy of antibacterial spray in the prevention of Chlamydia trachomatis infection at any site; (4) To investigate the efficacy of antibacterial spray in the prevention of Chlamydia trachomatis infection at oropharynx, urethra or anorectum; (5) To investigate the efficacy of antibacterial spray in the prevention of syphilis.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）过去三个月发生男男性行为者；
（2）年龄大于或等于18岁；
（3）基线检测口腔、肛门和尿道的淋球菌和沙眼衣原体均为阴性的男性；
（4）HIV、梅毒或其他性病感染者可以纳入该研究。

Inclusion criteria

(1) Men who have sex with men in the past 3 months;
(2) Men aged 18 years or above;
(3) At baseline, men tested negative for Neisseria gonorrhoeae and Chlamydia trachomatis at oropharynx, urethra and anorectum;
(4) Men co-infected with HIV, syphilis or other STIs are eligible.

排除标准：

（1）由于近期出行计划等原因不能完成随访；
（2）目前或长期需要使用抗生素；
（3）存在口咽健康问题不能耐受乳酸；
（4）在18周内不愿意使用喷雾；
（5）与已纳入参试者相识；
（6）同时参加其他试验研究。

Exclusion criteria：

(1) Unable to attend the scheduled visits because of travelling plan;
(2) Current or long-term use of antibiotics;
(3) Men with oral health problems who cannot tolerate lactic acid;
(4) Not willing to use the spray within 18 weeks;
(5) Men who know someone in their household who is currently enrolled in the study;
(6) Currently participating in the other clinical trials.

分配方案由独立统计学家通过随机数字表产生。参试者被1：1：1：1随机分配至试验组1、试验组2、安慰剂组1或安慰剂组2。分配方案由独立统计学家放入不透明的信封。

The allocation scheme was generated by an independent statistician using the random number table. Participants were 1:1:1:1 randomly assigned to experimental group 1, experimental group 2, placebo group 1 or placebo group 2. Allocation scheme was put into the opaque envelope by the independent statistician.

盲法：

试验采用双盲设计，实施干预者、结局测量者、数据收集者以及参试者不知晓真实分配方案。淋球菌或沙眼衣原体感染阳性者将由医生通知参试者，并告知其一线治疗方案。喷雾将在西安交通大学包装并编号，然后被送到参与的机构。由于试验组喷雾口味为略酸，与安慰剂组不同，我们通过将两种喷雾包装到相同的容器中来使喷雾盲法。尽管两种喷雾口味不同，因参试者之前未尝试过该种喷雾，故不知晓喷雾分配情况。每6周给予参试者2瓶500ml溶液瓶。参试者可以在任何时间点获取更多的溶液瓶，并记录提供每个参与者的额外溶液瓶的数量。

Blinding：

The trial uses a double-blind design, and the interveners, outcome measurers, data collectors, and participants are blinded to the real allocation scheme. Those who are positive for Neisseria gonorrhoeae or Chlamydia trachomatis infection will be notified by the doctor and informed of the first-line treatment scheme. The sprays will be packaged and numbered at Xi'an Jiaotong University before being sent to participating institutions. Since the experimental group spray tasted slightly sour, unlike the placebo group spray, we blinded the spray by packaging both sprays into the same container. Although the two sprays had different flavors, the participants had not tried the spray before and were unaware of the spray distribution. Participants are given 2 vials of 500ml solution every 6 weeks. Participants can obtain more solution at any time, and the number of additional solution provided to each participant was recorded.
