主要研究新型卡压、填充型双套囊支气管封堵器在胸科手术患者的应用。  

Objectives of Study：

The purpose of this study is to investigate the application of a new type of compression and filling double cuff bronchial occluder in patients undergoing thoracic surgery.

药物成份或治疗方案详述：

主要研究内容： 临床工作中，开胸的患者使用支气管封堵器比使用双腔管更加方便，我们科室在实际的使用中经常插单管后直接摆放侧卧位，然后直接盲探下封堵，效果良好。对于开右胸的手术，支气管封堵器就显得有些不太理想，因为右主支气管较短，右肺上叶支气管开口距离主气管太近，使用封堵器极难使右肺完全萎陷，特别是右肺上叶，这样不利于手术操作，从而影响了使用。本研究主要研究新型卡压、填充型双套囊支气管封堵器（已经上报专利申请）在胸科手术使用是否可以解决这些问题。封堵管的优点是手术结束后可以直接拔出封堵管，避免将双腔管拔出后再行更换单管，同时封堵管配合加强型气管导管可以极大的减少患者的损伤，术后声嘶、咽痛、勺状软骨脱位等并发症大大的低于使用双腔管者。本研究的目的是研究新型卡压、填充型双套囊支气管封堵器在临床工作中使用的安全性和有效性，同时探索该新型封堵管在不同身高，不同性别胸科患者使用规律，包括放置的成功率，放置技巧和深度的规律，以利于该新型封堵器的临床推广。 

Description for medicine or protocol of treatment in detail：

Main research contents In clinical work, it is more convenient for patients with thoracotomy to use the bronchial occluder than to use the double lumen tube. In actual use, our department often inserts a single tube, directly places the lateral position, and then directly blind explores to block. The effect is good. For the operation of opening the right chest, the bronchial occluder is not ideal, because the right main bronchus is short, and the opening of the right upper lobe bronchus is too close to the main bronchus, so it is very difficult to completely collapse the right lung with the occluder, especially the upper lobe of the right lung, which is not conducive to the operation, thus affecting the use. The purpose of this study is to investigate whether the use of a new type of compression and filling double cuff bronchial occluder (patent application has been submitted) in thoracic surgery can solve these problems. The advantages of the occluder tube are that the occluder tube can be pulled out directly after the operation, and the single tube can not be replaced after the double lumen tube is pulled out. At the same time, the occluder tube combined with the reinforced tracheal tube can greatly reduce the injury of the patients. The complications such as hoarseness, pharyngeal pain and spoon cartilage dislocation after the operation are much lower than those using the double lumen tube. The purpose of this study is to study the safety and effectiveness of the new type of compression and filling double capsule bronchial occluder in clinical work, and to explore the rules of the new type of occluder in chest patients of different height and gender, including the success rate of placement, placement skills and depth, so as to facilitate the clinical promotion of the new type of occluder. 

纳入标准：

行胸科择期手术及部分急诊手术（含开胸和胸腔镜手术），ASA分级I-II级，年龄18～75岁，体重45～100kg。

Inclusion criteria

ASA grade I-II, aged 18-75 years and weight was 45-100kg.

排除标准：

(1) 患有支气管扩张症、支气管脓肿、单侧肺患有感染性疾病等需要左右肺叶绝对隔离者；
(2) 与胸科医师沟通后胸科医师要求使用双腔管者；
(3) 研究者认为不适宜参加临床试验者。

Exclusion criteria：

(1) Patients with bronchiectasis, bronchial abscess, unilateral lung infectious diseases, etc. who need absolute isolation of left and right lobes;
(2) After communicating with the chest physician, the chest physician requires the use of double lumen tube; (3) Researchers think it is not suitable to participate in clinical trials.
