研究目的：

中风后抑郁（PSD）是中风后常见心理活动障碍，严重影响患者康复进程和生活质量。我们前期研究发现PSD患者常伴随胃肠道症状，且汉密尔顿抑郁量表（HAMD-17）评分中胃肠道症状因子变化明显，抑郁情绪与胃肠道症状之间存在相关性。目前，临床上针对PSD患者抑郁情绪及胃肠功能主要采取药物疗法，然而药物治疗存在明显副作用。“五音调神”法作为中国传统的康复技术作用于PSD患者抑郁情绪和胃肠功能的疗效及作用机制尚不清楚。因此，本研究旨在探讨“五音调神”法对轻中度PSD伴胃肠道症状患者抑郁情绪、胃肠功能，神经功能恢复、生活质量、肠道菌群改变的即刻-长期效应，将其嵌入临床路径，形成PSD中医心理康复处方。  

Objectives of Study：

Post-stroke depression (PSD) is a common mental activity disorder after stroke, which seriously affects patients' recovery and quality of life. Our previous study found that gastrointestinal symptoms often accompanied patients with PSD. The gastrointestinal symptoms factor in the Hamilton Depression Inventory (HAMD-17) score changed significantly, and there was a correlation between depressed mood and gastrointestinal symptoms. Currently, the clinical treatment for depression and gastrointestinal function of PSD patients mainly adopts pharmacological therapy. However, there are apparent side effects of pharmacological treatment. The efficacy and mechanism of the "Wu Yin Tiao Shen" therapy, a traditional Chinese rehabilitation technique, on the depression and gastrointestinal function of PSD patients are still unclear. Therefore, this study aims to investigate the effects of "Wu Yin Tiao Shen" therapy on the immediate and long-term effects of depression, gastrointestinal function, neurological recovery, quality of life, and intestinal flora change in patients with mild to moderate PSD with gastrointestinal symptoms.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）符合颅脑CT或MRI的卒中影像学诊断标准、抑郁症的中西医诊断标准；
（2）伴有胃肠道症状，HAMD-17单项胃肠道症状评分≥1分，且胃肠道症状继发于PSD；
（3）年龄18-75周岁，性别不限；
（4）PSD发生时间为脑卒中后2-36周；
（5）PSD严重程度为轻中度，即HAMD-17量表评分为8-24分；
（6）签署知情同意书。

Inclusion criteria

(1) Meeting the cranial CT or MRI diagnostic imaging criteria for stroke, as well as the Chinese and Western medical diagnostic criteria for depression.
(2) With gastrointestinal symptoms, a HAMD-17 score of 1 for a single gastrointestinal symptom, and secondary to PSD gastrointestinal symptoms
(3) Between the ages of 18 and 75, regardless of gender
(4) The onset of PSD is 2–36 weeks after the stroke.
(5) PSD severity is mild to moderate, i.e., a score of 8-24 on the HAMD-17 scale.
(6) Sign the form requesting informed consent.

排除标准：

（1）HAMD-17评分自杀倾向单项因子评分≥3分；
（2）生命体征不平稳或心、肝、肾有严重功能障碍者；
（3）伴有明显的认知障碍、失语和视听功能障碍不能完成评估者；
（4）近1年内有重大精神创伤史及酒精、药物依赖者；
（5）有原发性抑郁症和抗抑郁药物服用史者；
（6）伴有胃肠道器质性病变者；
（7）伴有痴呆、帕金森病、癫痫等可能影响PSD严重程度评估者；
（8）对音乐反感或不能接受，存在针刺治疗禁忌症的患者。

Exclusion criteria：

(1) HAMD-17 score for suicidal ideation with a single factor score of ≥3.
(2) Patients with fluctuating vital signs or severe cardiac, hepatic, or renal failure
(3) Patients with severe cognitive impairment, aphasia, or audiovisual dysfunction cannot complete the assessment.
(4) patients who have recently experienced significant trauma and are dependent on alcohol or drugs
(5) Patients on antidepressant medication who have a history of major depression.
(6) patients with organic gastrointestinal lesions
(7) Patients with dementia, Parkinson's disease, epilepsy, etc., may affect the assessment of the severity of PSD.
(8) Patients who are averse or unacceptable to music and for whom there are contraindications to acupuncture treatment should be excluded.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

按照课题研究方案制定《“五音调神”法对轻中度中风后抑郁患者抑郁情绪和胃肠功能的影响》临床病例报告表CRF，依据研究进程和填写说明完成填写。 试验结束双重录入后系统校对，数据的编码部分暂且保密，只有课题组对于数据的输入、阅读、修改的权限。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

The clinical case report from CRF was developed per the research protocol, "The Effect of the “Wu Yin Tiao Shen” therapy on Depression and Gastrointestinal Function in Patients with Mild to Moderate Post-stroke Depression," and completed following the research process and instructions for completion. After the double entry of the experiment, the system will be proofread, and the coding part of the data will be kept secret for the time being, and only the research group has the authority to input, read, and modify the data.
