研究目的：

在湖北省内14 个地级市（州）能够满足试验要求的医疗机构进行。主要目的是验证方案治疗组在降低冠心病合并2 型糖尿病患者的复合心血管事件（心血管死亡、非致死性心梗、非致死性脑卒中、心力衰竭住院、急性冠脉综合征再住院）发生率方面比常规治疗组更有效。另外通过引入基因组学数据，进一步从分子基因层面深入研究复合心血管事件的遗传机制，并探索遗传多样性对治疗效果的贡献及影响。同时探索建立省市级卫生健康行政部门领导，省级防治中心组织、市州级防治中心指导二级以上医疗机构分工协作的心血管疾病防治工作体系和有效机制，为制定大规模干预相关疾病的策略提供理论和试验依据，从而改进防治现状和效果，努力减轻疾病负担和社会负担。  

Objectives of Study：

It was carried out in medical institutions in 14 prefecture level cities (prefectures) in Hubei province that could meet the test requirements. The main purpose is to verify that the protocol treatment group is more effective than the conventional treatment group in reducing the incidence of composite cardiovascular events (cardiovascular death, non fatal myocardial infarction, non fatal stroke, hospitalization for heart failure, and readmission for acute coronary syndrome) in patients with coronary heart disease and type 2 diabetes. In addition, we will further study the genetic mechanism of compound cardiovascular events from the molecular genetic level, and explore the contribution and impact of genetic diversity on the treatment effect. At the same time, explore and establish a cardiovascular disease prevention and control system and effective mechanism under the leadership of provincial and municipal health administrative departments, organized by provincial prevention and control centers, and guided by municipal and state prevention and control centers, which provide theoretical and experimental basis for formulating strategies for large-scale intervention of related diseases, so as to improve the prevention and control status and effect, and strive to reduce the disease and social burden.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）40-75 岁，男女不限；
（2）确诊为冠心病合并2 型糖尿病者(造影确诊);
（3）低密度脂蛋白胆固醇>1.8mmol/L；
（4）签署了书面的知情同意书。

Inclusion criteria

(1) 40-75 years old, male or female;

(2) Patients with coronary heart disease complicated with type 2 diabetes (confirmed by angiography);

(3) Low density lipoprotein cholesterol >1.8mmol/l;

(4) Signed written informed consent.

排除标准：

（1）近一年内接受冠脉支架治疗;
（2）妊娠或哺乳期妇女；
（3）慢性收缩性心力衰竭失代偿期（NYHA 心功能分级≥3 级）、心
源性休克等严重心血管并发症患者；
（4）近1 年内发生过脑卒中患者；
（5）严重胃肠道疾病、肝肾功能衰竭患者；
（6）癌症患者或五年之内接受过放射治疗或化学治疗患者；
（7）患有艾滋病和肺结核等传染性疾病；
（8）住院或患有其他需要立即住院治疗的严重疾病者；
（9）不良嗜好者：使用毒品或者酗酒者（男性摄入酒精>80 克/天，
女性>40 克/天）；
（10）意识障碍、无法正常交流或任何精神疾病；生活不能自理或行
动不便者;
（11）预期寿命<12 个月者;
（12）实验室检测和临床表现异常，经研究者判断不适合参加的个体;
（13）不愿意参加试验，不愿改变目前的药物治疗方案。

Exclusion criteria：

(1) Received coronary stent treatment in recent one year;

(2) Pregnant or lactating women;

(3) Decompensated stage of chronic systolic heart failure (NYHA cardiac function rating ≥ 3), cardiac function

Patients with severe cardiovascular complications such as septic shock;

(4) Stroke patients in recent 1 year;

(5) Patients with severe gastrointestinal diseases and liver and kidney failure;

(6) Cancer patients or patients who have received radiotherapy or chemotherapy within five years;

(7) Suffering from infectious diseases such as AIDS and tuberculosis;

(8) Hospitalized or suffering from other serious diseases requiring immediate hospitalization;

(9) Bad addicts: drug users or alcoholics (men consume alcohol >80 g / day,

Women >40 g / day);

(10) Disturbance of consciousness, inability to communicate normally or any mental illness; You can't take care of yourself or walk in life

Those who are inconvenient to move;

(11) Life expectancy <12 months;

(12) Individuals with abnormal laboratory tests and clinical manifestations who are judged by the researcher to be unsuitable for participation;

(13) Unwilling to participate in the trial, unwilling to change the current drug treatment plan.
