研究目的：

1、进行围术期老年髋部骨折患者营养风险筛查和营养不良评定； 2、进行围术期老年髋部骨折患者营养支持治疗作用进行前瞻性、非随机同期对照研究  

Objectives of Study：

1 Malnutrition screening and assessment in elderly patients with hip fracture in the perioperative period 2 To evaluate the effect of nutritional support guided by Harris–Benedict in geriatric patients following surgery for hip fractures.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1) 年龄≥65 岁以上；
(2) 髋部骨折；
⑶ 进行手术治疗；
⑷ NRS 2002 评分≥3 分且MNA-SF 评分≤11 分；
⑸ 签署知情同意书的受试者。

Inclusion criteria

(1) age ≥ 65 years
(2) hip fracture
(3) choose surgical treatment
(4) the NRS-2002 score ≥3 and the MNA-SF score ≤11
(5) Informed consent is signed by all study participants

排除标准：

(1) 不能进食者；
(2) 受伤至就诊时间＞48 小时；
⑶ 肿瘤、结核等非外伤性因素导致的病理性骨折；
⑷ 合并其它部位的骨折或髋部开放骨折；
⑸ 骨折前已卧床；
⑹ 入院时已存在骨折相关并发症，如压疮、感染、深静脉血栓形成等；
⑺ 痴呆；
⑻ 合并严重内科系统疾病，包括慢性心衰（纽约分级≥2 级）、慢性肾功能不全（慢性肾脏病 4 期、5 期）、肝硬化、慢性肺病（如慢性阻塞性肺病等）、肿瘤、消化道疾病（如慢性胰腺炎、克罗恩氏病、溃疡性结肠炎）；
⑼ 入院前已开始口服营养液；
⑽ 术后 3 周、2 个月不能复查。

Exclusion criteria：

(1) aphagosis
(2) present to hospital > 48 hours after the injury
(3) pathological fracture
(4) multiplefractures or opened fracture
(5) long-term bedridden before the injury
(6) diagnosed as pressure sores, infection or venous thromboembolism at admission
(7) dementia
(8) in the presence of serious health problems, such as chronic heart failure (NYHA ≥ class 2 ), chronic kidney failure (CKD4, CKD5), cirrhosis, chronic lung diseases, tumors, chronic pancreatitis, crohn's disease and ulcerative colitis
(9) received oral nutritional supplements before the injury
(10) unable to complete follow-up examination

①研究者按照 Harris-Benedict 公式计算受试者每日所需热量；②研究者记录受试者每日进食食物种类、量，计算摄入的热量（包括前一日午餐、晚餐及当日早餐），并计算摄入蛋白质、脂肪、碳水化合物量；③计算热量差=Harris-Benedict 公式计算值-实际摄入热量；④将热量差换算为瑞高需求量，并提供给受试者。受试者可以当天一次喝完，也可以分次喝完；⑤如果受试者食物摄入热量≥Harris-Benedict 公式计算值，则当日不予以瑞高口服。
