研究目的：

验证 KMMUS 评分体系的准确性和实用性。优化 KMMUS 评分体系，统一手术标准，使 CMI 手术治疗规范化。  

Objectives of Study：

Verify the accuracy and practicability of kmmus scoring system. Optimize the kmmus scoring system, unify the surgical standards, and standardize the surgical treatment of CMI.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

第一部分：①MRI 提示小脑扁桃体下疝至枕骨大孔平面下距离≥3mm；②首次进行手术的患者。
第二部分：①MRI 提示小脑扁桃体下疝至枕骨大孔平面下距离≥3mm；②排外颅内其他疾病引起的临床症状；③首次进行手术的患者；④获得患者及家属的知情同意，愿意积极配合。

Inclusion criteria

Part I: ① MRI showed that the distance from cerebellar tonsillar hernia to the plane of foramen magnum was ≥ 3mm; ② Patients undergoing surgery for the first time.

Part II: ① MRI showed that the distance from cerebellar tonsillar hernia to the plane of foramen magnum was ≥ 3mm; ② Exclude clinical symptoms caused by other intracranial diseases; ③ Patients undergoing surgery for the first time; ④ Obtain the informed consent of patients and their families, and be willing to actively cooperate.

排除标准：

第一部分：①拒绝手术治疗的患者；②行钉棒内固定手术的患者；③因各种原因导致失访的患者。
第二部分： ①合并重大基础疾病，有手术禁忌症者；②患者及家属不愿意手术治疗或拒绝参加试验；③有智力低下、精神异常、语言表达能力有障碍的患者

Exclusion criteria：

Part I: ① patients who refuse surgical treatment; ② Patients who underwent nail rod internal fixation; ③ Patients lost to follow-up due to various reasons.

Part II: ① patients with major basic diseases and surgical contraindications; ② Patients and their families are unwilling to undergo surgical treatment or refuse to participate in the trial; ③ There are patients with mental retardation, mental disorder and language expression disorder

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

采集：数据收集包括人口学特征、临床表现、影像学特征、KMMUS 得分、手术方式，术后CCOS、mJOA、脊髓空洞张力指数与影像学情况等。实验组和对照组中所有数据均由神经昆明医科大学第二附属医院院内临床研究项目申报书14外科主治及以上职称医生和有经验的神经放射科医生的指导下完成收集并详细如实记录，填写 CRF表，最终由两人录入数据进行核实对比。管理：使用Epidate电子系统管理，项目实施过程中对每一个患者进行追踪管理，减少失访及数据丢失、错误，定期对数据进行核对检查。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Collection: data collection included demographic characteristics, clinical manifestations, imaging characteristics, kmmus score, operation method, postoperative CCOS, mJOA, spinal cord tension index and imaging. All data in the experimental group and the control group were collected and recorded in detail under the guidance of doctors with the title of chief surgeon or above and experienced neuroradiologists in the application form for clinical research projects in the hospital of the Second Affiliated Hospital of Kunming Medical University, and filled in the CRF form. Finally, the two entered the data for verification and comparison. Management: during the implementation of the project, track and manage each patient to reduce the loss of follow-up, data loss and errors, and check the data regularly.
