研究目的：

本课题拟开展一项前瞻性观察性临床研究，纳入约50名弥漫性难治型子宫腺肌病患者，采用改良版手术方法——切除子宫腺肌病病灶同时缝合固定LNG-IUS（商品名“曼月乐”）于宫腔内，比较手术前后月经量、痛经程度、血CA125值及子宫体积等变化，来验证该方案的有效性和可靠性。此研究以期通过长时间固定LNG-IUS来达到持续的治疗效果，为弥漫性难治型子宫腺肌病患者找到有效安全的保留子宫的手术方式，并为后续研究和临床运用的推广提供可行性建议及治疗依据。  

Objectives of Study：

In this project, a prospective observational clinical study is proposed, including about 50 patients with diffuse refractory adenomyosis, using an improved surgical method - excision of adenomyosis lesions and suturing and fixing LNG-IUS (trade name "Mirena") in the uterine cavity, comparing changes in menstrual volume, degree of dysmenorrhea, blood CA125 value and uterine volume before and after surgery to verify the effectiveness and reliability of the protocol. This study aims to achieve sustained therapeutic effect by fixing LNG-IUS for a long time, find an effective and safe surgical method for patients with diffuse refractory adenomyosis, and provide feasibility recommendations and treatment basis for follow-up research and clinical application.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.经过B超或MRI诊断为弥漫性子宫腺肌病或大病灶腺肌病，病灶直径大于5 cm，宫腔深度大于10 cm，并确认子宫腺肌病病灶深达内膜；
2.曾经应用单纯性药物治疗但效果不佳；或者曾经有腹腔镜切除子宫腺肌病病灶的经历，但治疗效果不理想；
3.年龄范围在20-45岁，并且2年内无生育要求；
4.充分知情且同意入组的患者。

Inclusion criteria

1. It was diagnosed as diffuse adenomyopathy or large focal adenomyopathy by B-ultrasound or MRI, with a focal diameter greater than 5 cm and uterine depth greater than 10 cm, and confirmed that the adenomyopathy was deep into the endometrium;
2. Used simple drug therapy but the effect is not good; Or have the experience of laparoscopic adenomyosis, but the treatment effect is not ideal;
3. The age range is 20-45 years old, and there is no fertility requirement within 2 years;
4. Fully informed and consenting patients.

排除标准：

1.同时合并妇科其它器官的疾病，比如子宫颈病变、卵巢肿瘤等；
2.合并妇科阴道炎症等放置节育环的相关禁忌症；
3.有孕激素治疗的相关禁忌症；
4.有其它内外科基础疾病而无法行手术治疗的情况；
5.不同意入组的患者。

Exclusion criteria：

1. Combined with diseases of other organs of gynecology, such as cervical lesions, ovarian tumors, etc.;
2. Contraindications related to placement of IUD with gynecological and vaginal inflammation;
3. There are contraindications related to progesterone therapy;
4. Patients with other basic medical and surgical diseases that cannot be treated by surgery;
5. Patients who did not agree to be enrolled.
