研究目的：

本项目组基于rGERD“肝胃失和”的根本病机，拟针对rGERD最常见的证型，开展疏肝和胃法治疗气郁痰阻型rGERD的平行随机对照临床研究，建立气郁痰阻型rGERD的中医规范治疗方案，有望通过发挥中医药优势，解决目前临床上的难题，并形成中西医广泛认同的、临床易于推广的气郁痰阻型难治性胃食管反流病治疗方案。同时融合内镜与临床症状、生存质量评分的研究思路，创新性运用胃肠动力检测技术，客观评价疏肝和胃法的疗效，为中药制剂的推广应用提供科学依据。  

Objectives of Study：

Based on the root of rGERD "liver-stomach disharmony", this project team intends to carry out a parallel randomized controlled clinical study of the method of soothing the liver and stomach in the treatment of rGERD with qi stagnation and phlegm obstruction, and establish a standardized treatment plan of traditional Chinese medicine for rGERD with qi stagnation and phlegm obstruction, which is expected to solve the current clinical problems by exerting the advantages of traditional Chinese medicine, and form a widely recognized treatment plan for refractory gastroesophageal disease with qi stagnation and phlegm obstruction, which is easy to be popularized in clinical practice. At the same time, integrate the research ideas of endoscopy with clinical symptoms and quality of life scores, innovatively use gastrointestinal motility detection technology, objectively evaluate the efficacy of the method of soothing the liver and stomach, and provide a scientific basis for the promotion and application of traditional Chinese medicine preparations.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、符合气郁痰阻型吐酸病（胃食管反流病）诊断标准。
2、参加本研究前连续服用8周双倍剂量PPI治疗胃食管反流病失败者。
3、年龄≥18周岁且≤70周岁，性别不限。
4、理解、同意参加本研究并签署知情同意书。

Inclusion criteria

1. Meet the diagnostic criteria of vomiting acid disease (gastroesophageal disease) with qi stagnation and phlegm obstruction.
2. Patients who failed to take double dose PPI for 8 weeks before participating in this study;
3. Age ≥ 18 years and ≤ 70 years, male or female;
4. Understand and agree to participate in this study and sign the informed consent form;

排除标准：

1、合并下列疾病之一者：消化性溃疡、胃食管和十二指肠手术史、卓－艾综合症、原发性食道动力疾患（如贲门失弛缓症，硬皮病，原发性食道痉挛）、上消化道恶性病变、冠心病、糖尿病、药物性食管炎患者。
2、长期服用抗凝药物及严重凝血功能障碍。
3、鼻腔畸形、近期有鼻咽部手术无法行24 h pH-阻抗监测、食管高分辨测压。
4、妊娠期或哺乳期妇女。
5、已知对本研究所使用的中药组成成分或富马酸伏诺拉生片存在过敏者。
6、入组前1个月参加过其他临床研究或正在参加其他临床研究者。
7、入组前1个月采用中草药不同剂型或富马酸伏诺拉生治疗者。

Exclusion criteria：

1. Patients with one of the following diseases: peptic ulcer, history of gastroesophageal and duodenal surgery, Zollinger-Ellison syndrome, primary esophageal motility diseases (such as achalasia, scleroderma, primary esophageal spasm), upper gastrointestinal malignant lesions, coronary heart disease, diabetes, drug-induced esophagitis;
2. Long-term use of anticoagulant drugs and severe coagulopathy;
3. Nasal deformity, recent nasopharyngeal surgery can not be 24 h pH-impedance monitoring, esophageal high-resolution manometry.
4. Pregnant or lactating women;
5. Known to be allergic to the composition of traditional Chinese medicine used in this study or vorazol fumarate tablets;
6. Participated in other clinical studies or are participating in other clinical investigators 1 month before enrollment;
7. Patients treated with different dosage forms of Chinese herbal medicine or vonoprazan fumarate 1 month before enrollment;
