研究目的：

1.建立多中心、大样本中国急性脊髓损伤标志物队列（队列数据库和生物样本数据库）；2.评估急性脊髓损伤的管理现状，包括院前急救，诊断，治疗及康复，通过与现行指南的比较，找出存在缺陷的流程；3.通过系统规范的预后因子分析框架，评估血生化指标和急性脊髓损伤严重程度及预后之间的关联强度，开发早期预测急性脊髓损伤严重程度及预后的血生化标志物；4.遵循医学影像人工智能研究清单（CLAIM），通过机器学习建模，系统的评估影像学（如DTI-MRI）表现和急性脊髓损伤严重程度及预后之间的关联强度，开发早期预测急性脊髓损伤严重程度及预后的影像学标志物。  

Objectives of Study：

1. Establish a multi-centre, large-sample Chinese marker cohort (cohort and biological sample database) of acute spinal cord injury; 2. Assess current management of acute spinal cord injury, including pre-hospital care, diagnosis, treatment and rehabilitation, and then identify deficient procedures by comparing them with current guidelines; 3. Assess the association of blood biochemical indicators with the severity and prognosis of acute spinal cord injury through systematic and standardized prognostic factor analysis framework, and then develop blood biochemical markers for early prediction of the severity and prognosis of acute spinal cord injury; 4. Systematically assess the association of imaging (such as DTI-MRI) manifestations with the severity and prognosis of acute spinal cord injury through following Checklist for Artificial Intelligence in Medical Imaging (CLAIM) and machine learning modelling, and then develop imaging markers for early prediction of the severity and prognosis of acute spinal cord injury.

纳入标准：

1.急性期（2周内）脊髓损伤患者；
2.年龄在15周岁及以上。

Inclusion criteria

1.Patients with spinal cord injury in acute stage (within 2 weeks);
2.Aged 15 years and above.

排除标准：

1.既往存在神经肌肉病变的患者；
2.肿瘤等其他因素引起的脊髓损伤；
3.存在精神疾病的患者；
4.不能配合完成病史采集、生物样本采样及随访的患者。

Exclusion criteria：

1.Patients with previous neuromuscular disease;
2.Spinal cord injury caused by tumors and other factors;
3.Patients with mental illness;
4.Patients who cannot cooperate with the completion of medical history collection, biological sample sampling and follow-up.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

(1)数据录入 研究者根据受试者的原始观察记录，将数据及时、完整、正确、清晰地载入病例报告表。经过监察员审核、签字后的调查表应及时送交临床研究数据管理员。录入采用相应的数据库系统双人双机录入，之后对数据库进行两遍比对，期间若发现问题及时通知监查员，要求研究者做出回答。他们之间的各种疑问及解答的交换应当采用疑问表形式，疑问表应保存备查。 (2)数据核查 当所有病例报告表经双份输入并核对无误后，由数据管理员写出数据库检查报告，其内容包括研究完成情况（含脱落受试者清单）、入选/排除标准检查、完整性检查、逻辑一致性检查、离群数据检查、时间窗检查、合并用药检查、不良事件检查等。 在审核会议上，由主要研究者、申办单位代表、监查员、数据管理员和统计人员对数据库检查报告中提出的问题做出决议，写出审核报告，数据库同时将被锁定。 (3)数据存档 病例报告表在按要求完成数据录入和核查后，按编号的顺序归档保存，并填有检索目录等，以备查考。电子数据文件包括数据库、检查程序、分析程序、分析结果、编码本和说明文件等，应分类保存，并有多个备份保存于不同磁盘或记录介质上，妥善保存，防止损坏。所有原始档案应按相应规定内的期限保存。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

(1)Data Entry According to the original records of the participants, the researcher copies the data into the form timely, completely, correctly, and clearly. The questionnaire that is reviewed and signed by the inspector should be sent to the research data manager in time. The data entry is conducted by two independent researchers with two computers, and then the database is compared twice. If any problems are found during this period, the inspector will be notified in time, and the researcher will be asked to answer. The questions and answers between them should be recorded in tables, which are kept for future auditing. (2) Data verification After all forms have been double-entered and verified, the research data manager will write a database inspection report. Database inspection report includes research completion status (including list of participants dropped out), selection/exclusion criteria, completeness, logical consistency, outliers, time window, drug combination, adverse events, etc. At the review meeting, the main researchers, representatives of the sponsoring unit, inspectors, data administrators, and statisticians make resolutions on the issues raised in the database inspection report, write out the review report, and the database will be locked at the same time. (3) Data archiving After completing the data entry and verification, the data archive form is archived and saved in the order of numbering, and the retrieval directory is filled in for auditing. Electronic data files include database, inspection program, analysis program, analysis results, code and description files, etc. It should be classified and saved, and multiple backups should be saved on different disks or recording media to properly save and prevent damage. All original files shall be kept within the corresponding time limit.
